From the Nurse:

Summer is almost here! Please encourage your


children to keep safety in mind. Always wear your
bicycle helmet. Be sure to use safety gear when
skateboarding.

Field trips
If your child is attending field trips please make sure that they dress appropriately for the
activities. Please ensure your child has their medications at school so they can attend field
trips with their class.

FORMS FOR 2017/2018 SCHOOL YEAR: New forms are needed yearly for
medication to be given in school as well as medical updates for children with health
concerns. Forms will be sent home. Have your child’s doctor complete the forms over the
summer and return them at the start of school in September.

PHYSICALS: Physicals are required for grades PK, K (if not in PK), 2, 4 and 7. If your
child had a physical done after Sept. 1, 2016, this can be submitted for the 2017-2018
school year. Forms will be sent home.

MEDICATION IN SCHOOL
If your child has any medication, an inhaler, etc. in the clinic, please pick it up on
the last day of school. Medication cannot be sent home with the children.
Medication not picked up on the last day of school will be disposed of.

Important Immunization Information for 5th and 6th Grade


Students

New York State requires a Tdap Booster shot and proof of a Varicella shot for all students
entering 6th grade. Letters were sent home if your child needs this shot done. This shot
should be given once your child turns 11. Your child can be excluded from starting 6th
grade in Sept. if this is not done. If your child turns 11 after the start of the school year in
Sept., the shot must be done after their birthday.

All children entering 7th grade in Sept 2016 are required to have the meningococcal
vaccine per New York State. Parent please check with your child’s Doctor. Even kids who
have had a shot before may need a booster. For more information, you can learn more about
the Disease and vaccine at health.ny.gov/immunize.
