1. agree to fulfill the following rules:

a. fully participate in the Online Training on Teachers’ Competence Improvement from
5 April—5 July 2021 u ​ sing Altissia learning platform (minimum 64 hours);
b. take the placement test on the learning platform between 19 February—5 March
2021 to determine my current language skill level;
c. take the final test on the learning platform between 28 June—5 July 2021 to evaluate
my language skill level after the training;
d. accept any results obtained through the placement and final test;
e. fill out the training evaluation form; and
f. accept SEAMEO QITEP in Language and Altissia’s decision regarding the completion
of the training.

2. will conform to the aforementioned rules. Should I fail to obey the rules, I ​agree to:
a. lose an opportunity in receiving certificate of participation and/or achievement​; and
b. accept that SEAMEO QITEP in Language to deliver an official letter to my institution
about my inability to complete the training.

I hereby declare that above statement is true and to be used

accordingly.

1) Certificate of participation will only be given to teachers who fully participate in the training for a
minimum of 64 hours via learning platform and complete the training evaluation form.
2) Certificate of achievement will only be given to teachers who fully participate in the training for a
minimum of 64 hours via learning platform and success to increase their language skills for one or
 more level based on the placement and final test result as well as complete the training evaluation
form.
