ABOUT THE PT INCO MAJOR HAZARD STANDARDS

These Standards are designed to address the major safety hazards at PT Inco. For each major
hazard they define the full range of available controls that could be implemented to address all
aspects of the hazard in the context of industry best-practice.

Compliance with these Standards comprises a long-term management goal for the site, as does
our commitment to zero injury for all workers. It is not expected that all areas of operation could
be in complete compliance with all aspects of each Standard, just as it is not expected that all
risk can be eliminated from any mining and processing operation.

However, the concept of best practice includes a commitment to ongoing improvement, and
these Standards provide a means of measuring and guiding that progress. A baseline audit will
be conducted in 2006 to measure compliance with all 18 Major hazard Standards, and this in
turn will form the basis for improvement plans in all Departments that will result in progressive
reduction of risk of injury from accidents.

Standar ini dirancang untuk mengatur bahaya keselamatan utama di PT Inco. Untuk tiap
bahaya utama yang sudah mempunyai pengendalian dengan jangkauan penuh yang dapat
diterapkan untuk mengatur semua aspek bahaya dalam konteks praktek-praktek terbaik dalam
industri.

Pematuhan terhadap standar ini terdiri dari tujuan manajemen jangka panjang di lokasi,
sebagaimana halnya komitmen kami untuk menuju nihil cidera bagi pekerja. Tidaklah
diharapkan bahwa semua area operasi dapat seluruhnya mematuhi semua aspek dari setiap
standar, sebagaimana yang tak diharapkan bahwa semua resiko dapat dieliminasi dari setiap
operasi penambangan dan pengolahan.

Namun demikian, konsep tentang praktek-praktek terbaik termasuk merupakan suatu komitmen
untuk terus menerus mengupayakan peningkatan, dan Standar ini memberikan suatu cara untuk
mengukur dan memberikan pedoman kemajuan itu. Suatu audit sebagai garis dasar akan
dilakukan pada tahun 2006 untuk mengukur kepatuhan terhadap seluruh 18 Standar Bahaya
Utama, dan ini pada gilirannya akan membentuk suatu basis dalam melakukan peningkatan
yang bakal memberikan hasil pengurangan secara progressiv terhadap resiko cedera yang
terjadi akibat kecelakaan.

PURPOSE / TUJUAN
To ensure that all vehicles and mobile equipment in use at PT Inco are operated in a safe manner.

Memastikan bahwa semua kendaraan dan alat berat yang digunakan di PT Inco dioperasikan dengan cara yang benar.

SCOPE / RUANG LINGKUP


This Standard applies to all PT Inco and Contractor personnel driving vehicles or mobile equipment for the purpose of
work at PT Inco.

Standar ini berlaku pada semua karyawan PT Inco dan karyawan Kontraktor yang mengemudikan kendaraan atau alat
berat untuk suatu tujuan pekerjaan di PT Inco.

REQUIREMENTS / PERSYARATAN

1. HAZARD IDENTIFICATION, RISK ASSESSMENT AND CONTROL / IDENTIFIKASI BAHAYA, PENILAIAN


RISIKO DAN PENGENDALIANNYA

Objectives

‰ To apply a risk management process to enable the identification and control of hazardous driving practices.
‰ To require the development of traffic control plans.
‰ To ensure the PT Inco change management process is applied.

Tujuan

‰ Menerapkan suatu proses pengelolaan risiko agar dapat mengidentifikasi dan mengendalikan praktik mengemudi
yang berbahaya.
‰ Mengharuskan pengembangan rencana pengendalian lalu lintas.
‰ Memastikan diterapkannya proses "change management“ PT Inco.

Outcome Accountability Tool

1.1 Traffic Management / Manajemen Lalu Lintas PTI Area Traffic


General Management
A Traffic Management Plan must be established for all PT Inco lease areas that Managers Plan
will be accessible and kept up to date.

Additions or alterations to this plan must be based on a formal risk assessment


that evaluates the impact to safety of vehicle, mobile equipment and pedestrian
interactions.

The Traffic Management Plan shall define:

1. Road signage.
2. Road markings.
3. Other controls required for the safe interaction of all vehicles, mobile
equipment and pedestrians.

The plan must meet requirements stipulated in the Mine Ministry Regulation
(Kep-men 555K) in Article 144 - Safe Working Procedures (section 1 – 4),
(Attachment A).

Traffic Management Plan yang mudah diakses dan selalu diperbarui harus
ditetapkan untuk semua wilayah operasi PT Inco.

Penambahan atau perubahan terhadap rencana ini harus berdasarkan pada


penilaian risiko formal yang mengevaluasi dampak keamanan dari kendaraan,
alat berat, dan hubungannya dengan pejalan kaki.

Traffic Management Plan harus menetapkan:


1. Rambu jalan.
2. Marka jalan.
3. Pengendalian alat lain yang diperlukan untuk membentuk hubungan yang
aman dari semua kendaraan, alat berat, dan pejalan kaki.

Rencana tersebut harus memenuhi ketentuan yang ditetapkan pada Keputusan


Menteri Pertambangan (Kep-men 555K) dalam Pasal 144 – Prosedur Kerja yang
Aman (bagian 1- 4), (Lampiran A).

1.2 Hazard Review / Pengkajian Bahaya General


Manager EHS
A review of incidents and accidents involving vehicles, mobile equipment and
pedestrians shall be conducted each month.

This review must assess:

1. Trends in accidents and incidents.


2. The adequacy of current controls.

3. Any required changes to current controls or need for new controls.

Pengkajian mengenai insiden dan kecelakaan yang melibatkan kendaraan, alat


berat dan pejalan kaki harus diadakan tiap bulan.

Peninjauan kembali ini harus menilai:

1. Pola kecenderungan dalam kecelakaan dan insiden.

2. Apakah sistem pengendalian saat ini sudah memadai.

3. Setiap perubahan yang diperlukan terhadap pengendalian saat ini atau


apakah diperlukan sistem pengendalian yang baru.

1.3 Controls / Sistem Pengendalian PTI Area Monthly Report


General on Vehicle and
Controls must be developed and implemented to address the hazards identified Managers and Mobile

1.4 Change Management / Manajemen Perubahan PTI Area PT Inco
General Change
The PT Inco Change Management Procedure must be followed to authorise Managers Management
changes to the Traffic Management Plan, PTI Drivers Manual or operating Procedure
practices of vehicles and mobile equipment.

Prosedur “Change Management” PT Inco harus diikuti untuk mengesahkan


perubahan terhadap Traffic Management Plan, Drivers Manual PT Inco atau
praktik pengoperasian kendaraan dan alat berat.

2. SELECTION, TRAINING, COMPETENCY AND AUTHORISATION / SELEKSI, PELATIHAN,


KOMPETENSI DAN WEWENANG

Objectives

‰ To ensure the recruitment process includes criteria for medical fitness of personnel required to operate vehicles
and mobile equipment.

‰ To ensure personnel required to operate vehicle and mobile equipment are competent.
‰ To define the process that must be followed to obtain authorisation to drive vehicles and operate mobile
equipment on PT Inco lease areas.

Tujuan

‰ Memastikan proses rekrutmen mencakup kriteria untuk kesehatan medis bagi karyawan yang ditugaskan untuk
mengoperasikan kendaraan dan alat berat.

‰ Memastikan karyawan yang ditugaskan berkompeten untuk mengoperasikan kendaraan dan alat berat.
‰ Menentukan proses yang harus diikuti untuk mendapatkan wewenang untuk mengendarai kendaraan dan
mengoperasikan alat berat pada wilayah operasi PT Inco.

2.1 Recruitment / Rekrutmen Director HR and SOP


Contractor Recruitment
The recruitment process used by PT Inco and all Contractor Company’s shall Company

A training and competency system shall be implemented to ensure that all Company Equipment
persons at PT Inco have adequate knowledge and skills pertaining to the Managers Skills Matrix
risks being being addressed by the MHS and the associated controls. This shall
include:

1. Mandatory training and licensing requirements for certain types of work


that may expose persons to the major hazards. This shall include:

ƒ General Inductions for all employees, and site specific


inductions for certain employee groups based on risk (that

include reference to the major hazards at PT Inco and the
MHS system and related controls).

ƒ A program of Basic Safety Training for persons involved in


working at heights, confined space, rigging (and other work
based on risk).

ƒ A program of mandatory licensing for certain types of work


requiring specialized skill sets (e.g. light vehicle operation).

2. Department-based training to meet specific needs based on risk.

3. Competency in these defined training requirements assessed by


qualified workplace assessors and recorded in a system that allows
ready verification and tracking of workplace competencies.

Specifically (where applicable) training shall be provided for drivers and


operators of each vehicle and mobile equipment type, on:

¾ Authorisation required to operate vehicles and mobile equipment.


¾ Equipment service inspection.
¾ Driving / operating procedures and techniques.
¾ Specific skills for hazardous tasks (eg land clearing, dozing on benches
without berms etc).

Workplace assessments for vehicle and mobile equipment operators shall be


conducted.

Workplace assessments shall be conducted by Workplace Assessors who have


been trained in assessment techniques and either:

o Possess relevant vehicle and mobile equipment operational


experience, or

o Utilise a specialist who possess the relevant technical


knowledge

yang dapat membahayakan seseorang. Pelatihan ini harus termasuk:

ƒ Induksi Umum untuk semua karyawan, dan induksi khusus


berdasarkan lokasi untuk kelompok karyawan tertentu
berdasarkan risiko (yang termasuk referensi terhadap bahaya-
bahaya utama di PT Inco dan sistem MHS dan kontrol-kontrol
terkait).

ƒ Program Pelatihan Keselamatan Dasar untuk orang yang


bekerja di ketinggian, ruang tertutup, rigging/tali-temali (dan
pekerjaan lain berdasarkan risiko).

ƒ Program pengaturan lisensi wajib untuk jenis pekerjaan


tertentu yang memerlukan serangkaian keahlian khusus
(misalnya operasi kendaraan ringan).

2. Pelatihan berdasarkan departemen untuk memenuhi kebutuhan

tertentu (berdasarkan risiko).

3. Kompetensi dalam persyaratan pelatihan tertentu ini dinilai oleh penilai


tempat kerja yang berkualifikasi, dan dicatat dalam sistem yang
memungkinkan verifikasi kesiapan dan pelacakan kompetensi di tempat
kerja.

Secara khusus (bila mungkin) harus diberikan pelatihan bagi pengemudi dan
operator setiap jenis kendaraan dan alat berat, mengenai:

¾ Izin atau otorisasi yang diperlukan untuk mengoperasikan kendaraan


dan alat berat.
¾ Inspeksi fungsi alat.
¾ Prosedur dan teknik mengemudi / mengoperasikan.
¾ Keahlian khusus untuk tugas berbahaya (misalnya pembukaan lahan,
membuldozer tepian bench tanpa pembatas dll).

Harus dilakukan penilaian tempat kerja untuk para operator kendaraan dan alat
berat.

Penilaian tempat kerja ini harus dilakukan oleh Penilai Tempat Kerja (Workplace
Assessors) yang telah terlatih dalam teknik penilaian dan:

o Memiliki pengalaman operasional kendaraan dan alat berat


yang relevan, atau

o Menggunakan spesialis yang memiliki pengetahuan teknis


yang relevan

2.3 Competency / Kompetensi PTI Area SOP High


General Hazard
All high hazard tasks requiring a high level of operator skill must have a Managers and Equipment
Standard Operational Procedure (SOP) and operator competency criteria Contractor Operations
developed to provide the operator with consistent training and work method. Company

These SOP’s and assessment criteria must be consistent with the requirements
outlined in the Mine Ministry Regulations and PTI Drivers Manual (refer to
Section 4.1).

Note: Examples include: land clearing, dozing on benches without berm,


haulage operations, forklift and loader operations.

harus mempunyai Standard Operational Procedure (SOP / Prosedur
Pengoperasian Standar) dan kriteria kompetensi operator, yang disusun untuk

dijabarkan dalam Keputusan Menteri Pertambangan dan Drivers Manual PT Inco
(lihatlah Bagian 4.1).

Catatan: Contoh-contohnya meliputi: perataan tanah di tepian tanpa tanggul,


aktivitas pengangkutan, pengoperasian forklift dan loader.

lease areas.
The process controlling the issuing and authorisation of PT Inco Drivers
Licenses must be defined in a standard operating procedure.

All personnel shall be required to be re-assessed for PT Inco Drivers Licences


every two years by an authorised Workplace Assessor.

3. COMMUNICATION AND AWARENESS / KOMUNIKASI DAN KESADARAN BAGI KARYAWAN

Objectives

‰ To ensure all changes to vehicles and mobile equipment operation and road rules are documented and
communicated to personnel.
‰ To ensure all personnel maintain awareness of operational practices and road rules.

Tujuan

‰ Memastikan semua perubahan dalam pengoperasian kendaraan dan alat berat dan peraturan jalan
didokumentasikan dan dikomunikasikan ke karyawan.
‰ Memastikan semua karyawan memiliki kesadaran yang baik mengenai praktik operasional dan peraturan di jalan.

3.1 Communication / Komunikasi PTI General

2. Information regarding haul roads / traffic routes / emergency pick up points.
3. Requirements related to operating conditions / traffic rules.
4. Specific skills and/or procedures for hazardous work (eg land clearing).

4. SPECIFICATIONS / SPESIFIKASI

‰ To ensure PTI Drivers Manual are consistent with Indonesian Traffic and Mine Ministry Regulations.
‰ To define specific rules for driving in active operational areas at PT Inco.
‰ To outline additional driving or operating rules that must be followed.

Tujuan

‰ Memastikan Traffic Rules PT Inco konsisten dengan Peraturan Lalu Lintas Indonesia dan Keputusan Menteri
Pertambangan.
‰ Menentukan peraturan tertentu untuk mengemudi dalam wilayah operasional aktif di PT Inco.
‰ Menjabarkan peraturan mengemudi atau peraturan pengoperasian tambahan yang harus diikuti.

4.1 PTI Drivers Manual / Traffic Rules PT Inco General PTI Drivers
Manager EHS Manual
A set of rules shall be developed and recorded in the PTI Drivers Manual
Manual.

The PTI Drivers Manual must comply with the Indonesian Traffic Regulations,
Undang – undang Lalu Lintas no 14/1993 and include all rules governing the
driving of all vehicles and mobile equipment at PT Inco.

Sebuah sistem peraturan akan dikembangkan dan dicatat dalam Drivers Manual
PT Inco.

Drivers Manual PT Inco harus sejalan dengan Peraturan Lalu Lintas Indonesia,
Undang – undang Lalu Lintas no 14/1993 dan mencakup semua peraturan yang
mengatur lalu lintas semua kendaraan dan alat berat di PT Inco.

4.2 Mine Ministry Regulations / Keputusan Menteri Pertambangan PTI Area SOP Mobile

Standard Operating Procedures and operator competency training shall ensure Managers and Operation
all personnel operating mobile equipment follow the regulations listed in the Mine Contractor
Ministry Regulation (Kep-men 555K) (Attachment A). Company

7.2 Emergency Services / Bantuan Saat Keadaan Darurat General

The PT Inco Fire Emergency Services (FES) must maintain the capability to deal
effectively with any incident or accident involving vehicles or mobile equipment.

This capability should include provision for the appropriate equipment suitable
for the controlled release of personnel from entrapment including:

1. Vehicles.
2. Static equipment and machinery.
3. Heavy mobile equipment, and
4. Aircraft.
Fire Emergency Services (FES) PT Inco harus mempertahankan
kemampuannya agar secara efektif mampu mengatasi setiap insiden dan

7.3 Emergency Vehicles Proceeding to an Emergency / Kendaraan General

The driver of an emergency vehicle is responsible for the safe passage of that
vehicle along a roadway and must obey the road traffic regulation and rules
outlined in the PTI Drivers Manual.

The driver of an emergency vehicle is permitted under Article 83 - Indonesian


Traffic Regulation, to exceed the sign posted speed limit in an emergency, but
only if it is safe to do so.

Undang – Undang RI. NO. 14 Tentang Lalu Lintas & Angkutan Jalan 1993
Article 83 - In a specific (emergency) situations the requirement of maximum or
minimum speed limits as stated in article 80, 81 and 82 is not applicable.

If a driver of an emergency vehicle must exceed the sign posted speed limit the
following conditions shall apply:

1. It is a declared emergency and the vehicle is travelling displaying visual


and audible warning signals.
2. The sign posted speed limit is not exceeded by more that 30 kph.
3. The speed limit may be exceeded when observance of those provisions is
likely to hinder the saving of life, extinguishing a fire or preventing a
criminal act.

Pengemudi kendaraan darurat bertanggung jawab atas keamanan perjalanan


kendaraan tersebut di sepanjang jalan dan harus mentaati peraturan lalu lintas
jalan dan Buku Manual Pengemudi PT Inco .

Sesuai Pasal 83 – Peraturan Lalu Lintas Indonesia, pengemudi kendaraan


darurat diizinkan untuk melebihi rambu batas kecepatan dalam keadaan darurat,
tetapi hanya apabila keadaannya aman untuk dilakukan.

Undang – Undang RI. NO. 14 Tentang Lalu Lintas & Angkutan Jalan 1993 Pasal
83 – Dalam keadaan tertentu (darurat) ketentuan batas kecepatan maksimum
atau minimum seperti yang disebutkan pada pasal 80, 81 dan 82 tidak berlaku.

Apabila seorang pengemudi kendaraan darurat harus melampaui rambu batas


kecepatan, kondisi berikut ini harus diterapkan:

1. Keadaan sudah dinyatakan darurat dan kendaraan berjalan dengan


menunjukkan tanda peringatan visual dan audio.
2. Tidak melampaui rambu batas kecepatan lebih dari 30 km/jam.
3. Batas kecepatan dapat dilampaui jika pematuhan ketentuan itu dapat
menghalangi penyelamatan nyawa seseorang, pemadaman kebakaran
atau pencegahan tindakan kriminal.

ATTACHMENT A – DECREE OF THE MINISTER OF MINES AND ENERGY
Number: 555.K/26/M.PE/1995 on General Mine Occupational Health and Safety

Article 142 – Driver Qualification and Obligations


Pasal 142 - Persyaratan Dan Kewajiban Pengemudi
(1) A person may operate a vehicle at a mine where the following requirements have been fulfilled:
a. Must be no less than 18 years of age;
b. Must have been appointed by the Technical Mine Manager to drive specific vehicles, and
c. Must have been given training and have been declared a competent driver by the Technical Mine
Manager.
Seseorang dapat mengemudikan kendaraan di tambang, apabila memenuhi persyaratan sebagai berikut :
a. berusia sekurang-kurangnya 18 tahun.
b. ditunjuk oleh Kepala Teknik Tambang untuk mengemudikan kendaraan tertentu, dan
c. telah mendapatkan pelatihan dan dinyatakan mampu mengemudi oleh Kepala Teknik Tambang

(2) A driver of any vehicle equipped with a tipping body shall ensure that if the body is raised for any purpose
other than routine unloading it shall be secured by the independent devices provided.
Pengemudi kendaraan yang dilengkapi dengan bak penumpah (tipping body) harus menjamin bahwa
apabila baknya diangkat untuk suatu tujuan selain dari membongkar muatan yang rutin, telah diamankan
dengan alat pengaman yang terpisah.

(3) The driver shall make use of any equipment provided at the unloading dock to prevent the said vehicle from
overturning, rolling or moving.
Pengemudi harus menggunakan alat yang disediakan pada tempat pembongkaran untuk mencegah
kendaraan tersebut terbalik, terguling atau bergerak.

(4) Any driver at any mining operation shall comply with the traffic signs as stipulated by the Technical Mine
Manager.
Semua pengemudi pada setiap kegiatan usaha pertambangan harus mentaati rambu-rambu lalu lintas yang
telah ditetapkan oleh Kepala Teknik Tambang.

(5) Before leaving the vehicle, the driver shall ensure that the vehicle is turned off and locked and safe thus
preventing accidental motion.
Sebelum meninggalkan kendaraannya pengemudi harus yakin bahwa kendaraannya sudah dimatikan
terkunci serta aman sehingga tidak dapat dijalankan atau secara tak sengaja berjalan.

(6) Prior to travelling along descending roads, the vehicle shall always be driven slowly and using the lowest
gear at all times, except where the said vehicle is being towed.
Bila melalui jalanan yang menurun, kendaraan harus selalu dijalankan dengan perlahan menggunakan gigi
rendah setiap waktu, kecuali apabila kendaraan tersebut digandeng.

(7) A driver shall not set any vehicle in motion before giving audible warning and ensuring that no person is in
the danger area of the vehicle.
Pengemudi sebelum menjalankan kendaraannya harus memberi tanda bunyi dan yakin tidak ada orang
disekitar kendaraannya.

(8) At the beginning of every work shift every driver shall undertake an inspection of the outer parts of the
vehicle and test the controls and particularly the braking capability.
Pada saat memulai gilir kerja setiap pengemudi harus melakukan pemeriksaan bagian-bagian luar dari
kendaraannya dan mencoba kerja alat pengendali terutama kemampuan rem.

Article 144 – Safe Working Procedures
Pasal 144 - Cara Kerja Yang Aman
(1) Kepala Teknik Tambang shall organize traffic routes in the mine and install necessary traffic signs to inform
the drivers of:
a. Traffic direction;
b. Speed limit;
c. Vehicle’s maximum height;
d. Incline and decline; and
e. “Parking” and “No Parking” areas and other matters related to transportation system safety.
Kepala Teknik Tambang harus mengatur arus lalu-lintas di pertambangan dan memasang tanda lalulintas
yang perlu untuk memberitahukan para pengemudi tentang :
a. arah lalu-lintas;
b. batas kecepatan;
c. batas tinggi kendaraan;
d. tanjakan/turunan; dan
e. “daerah parkir” dan “tidak boleh parkir” dan hal lain yang berhubungan dengan keselamatan sistem
pengangkutan.

(2) Traffic direction at loading, unloading and dumping points shall be one way.

Pada pekerjaan memuat, membongkar dan menumpahkan muatan, arus lalu lintas harus dibuat searah.

(3) Overtaking is allowable only at designated locations.

Pengemudi dapat mendahului kendaraan lain pada jalan yang telah ditetapkan.

(4) Mine workers who are permitted to walk or present at haul road, or loading and unloading points shall wear
loud fluorescent vest.
Pekerja tambang, yang diizinkan berjalan atau berada pada jalan angkutan atau pada tempat pemuatan
dan pembongkaran harus memakai rompi pantul (fluorescent) dengan warna yang menyolok.

Article 145 – Stockpiles


Pasal 145 - Pekerjaan Penimbunan
(1) Whenever waste or minerals are stockpiled, such work shall be supervised to ensure that the stockpiling
vehicle remains only in designated areas.
Pada setiap menimbun limbah atau bahan galian, pekerjaan tersebut harus diawasi sehingga kendaraan
yang menimbun hanya berada pada tempat yang ditetapkan.

(2) Over edge tipping by vehicles should be avoided and a berm or heap of material maintained at the tip
edge.
Kendaraan yang menimbun melebihi tepi tebing harus dihindarkan, tanggul atau onggokan bahan yang
ditimbun, harus tetap ada pada batas tepi penimbunan.

(3) Where over edge tipping is permitted stop block should be provided to prevent vehicles from overturning or
over running the tip edge.
Bila penimbunan di tepi tebing diizinkan, tanggul pengaman harus dipasang untuk menghindarkan
kendaraan tersebut terguling atau melewati tepi tebing timbunan.

(4) Stockpiling work shall be supervised by a stockpiling supervisor who shall designate stockpiling sites,
supervise vehicles moving in reverse and shall be responsible for the inspection of the sturdiness of the
stockpiling cliff edge.

Article 145 – Stockpiles
Pasal 145 - Pekerjaan Penimbunan
Pekerjaan penimbunan harus diawasi oleh pengawas penimbunan yang menetapkan tempat-tempat
penimbunan, mengawasi kendaraan yang sedang mundur dan bertanggung jawab dalam pemeriksaan
kemantapan tepi tebing penimbunan.

(5) Stockpiling work shall be avoided when dark or at times of an unclear field of vision except where lighting is
provided which guarantees safety and a clear field of vision.
Pekerjaan penimbunan harus dihindarkan pada waktu gelap atau jarak pang yang kurang jelas kecuali
dilengkapi dengan lampu penerangan yang menjamin keselamatan dan jarak pandang yang cukup jelas.

(6) The driver shall remain in the vehicle at the stockpiling area.

Pengemudi harus tetap berada dalam kendaraan pada daerah penimbunan.

(7) Only stockpiling PTI Supervisors shall be permitted to be at the stockpiling area and the said PTI
Supervisors shall bear a brightly coloured fluorescent coat.
Hanya pengawas penimbunan yang diperbolehkan berada di daerah penimbunan dan pengawas tersebut
harus memakai rompi pantul (fluorescent) dengan warna yang menyolok.

Article 251 – Prohibitions Against (Passengers)


Pasal 251 - Larangan Membawa Penumpang
(1) The operator shall prohibit any person from being on any earth moving equipment in operation, except for
the purposes of inspection, supervision, maintenance, repair work or upon the instruction of an authorised
trainer.
Operator wajib melarang setiap orang berada pada alat pemindah tanah yang sedang bekerja, kecuali
untuk kepentingan pemeriksaan, pengawasan, pemeliharaan, perbaikan atau atas instruksi pelatih yang
berwenang.

(2) It is prohibited for any person to board or to alight from any earth moving equipment which is still operating.

Dilarang seseorang naik ke atau turun dari alat pemindah tanah yang sedang beroperasi.

Article 250 – Operator Requirements


Pasal 250 – Persyaratan Operator
(1) Operators of earth moving equipment at any mining operation shall meet the following requirements:
a. Be no less than 21 years of age;
b. Have been declared both mentally and physically in good health by a doctor; and
c. Be in possession of an operating license by the Chief Mine Inspector or by any other authorised
employee on behalf of the Technical Mine Manager.
Operator alat pemindah tanah pada kegiatan usaha pertambangan harus memenuhi persyaratan sebagai
berikut :
a. telah berusia tidak kurang dari 21 tahun;
b. telah dinyatakan sehat baik mental maupun fisik oleh dokter, dan
c. telah memiliki surat keterangan hak mengoperasikan yang dikeluarkan oleh Kepala Teknik Tambang
atau oleh petugas lain yang berwenang atas nama Kepala Teknik Tambang.

(2) The operating license as required by paragraph (1) letter c may only be granted after the person applying
has successfully passed an operation examination carried out by the mining company concerned.
Surat keterangan hak mengoperasikan sebagaimana dimaksud dalam ayat (1) huruf c hanya dapat
diberikan setelah seseorang lulus ujian mengoperasikan alat pemindah tanah yang diselenggarakan oleh
perusahaaan pertambangan yang bersangkutan.

Article 250 – Operator Requirements

(3) The operating license shall only apply in the mining operations area where the said document was granted.

keterangan hak mengoperasikan tersebut diberikan.

Article 252 – Parking Earth Moving Equipment


Pasal 252 – Parkir Alat Pemindah Tanah
(1) It is prohibited to leave unattended any earth moving equipment except when the parking brake has been
applied, the bucket or knives have been lowered to ground level and the machine turned off.
Dilarang meninggalkan alat pemindah tanah, kecuali rem arker telah dipasang, ember atau pisau telah
diturunkan ke tanah dan mesin dimatikan.

(2) When earth moving equipment is parked on an incline, chocks shall be used under the wheels or the said
equipment shall be directed toward the roadside or embankment and the bucket and knives shall be
lowered to ground level.
Apabila alat pemindah tanah arker di tempat yang miring maka harns diganjal atau diarahkan ke tanggul
atau rusuk jalan dan ember atau pisau harus diturunkan ke tanah.

(3) When electrically operated earth moving equipment is to be left unattended, the master switch shall be
turned off and all of the controls shall be set to neutral and the parking brakes shall be applied.
Apabila alat pemindah tanah yang digerakkan tenaga listrik akan ditinggalkan, sakelar induk harus
dimatikan dan semua alat pengendali dalam keadaan netral serta rem arker harus dipasang.

(4) When any earth moving equipment is parked at any place which may be dangerous to other traffic the
parking lights shall be turned on as well as other hazard indicators.
Apabila alat pemindah tanah sedang arker di tempat yang dapat menimbulkan bahaya terhadap lalulintas
kendaraan lain maka lampu arker harus dinyalakan atau memasang tanda peringatan lainnya.

Article 254 – Equipment


Pasal 254 - Alat
(1) The soil conditions at the location where the earth moving equipment is operating shall be adequately
strong and in a safe and stable condition. To prevent accidental movement during loading, the loader shall
be secured with safety chocks.
Kondisi tanah tempat alat pemindah tanah beroperasi harus cukup kuat dan dalam kondisi aman serta
stabil, untuk mencegah gerakan yang tidak disengaja pada saat melakukan pemuatan, alat pemuat harus
diganjal balok pengaman.

(2) The area within the work radius of any earth moving equipment shall be free from obstructions and is
prohibited for any person to be within the said area.
Daerah dalam radius kerja alat pemindah tanah harus bebas dari rintangan dan dilarang orang berada
dalam daerah tersebut.

(3) Prior to operating any earth moving equipment, the operator shall first sound an audible warning.

Sebelum menjalankan alat pemindah tanah, operator harus terlebih dahulu memberikan tanda bunyi
sebagai peringatan.

(4) Earth moving equipment shall be operated in accordance with the manufacturer’s instructions with regard
to the work load, speed, engine Rpm’s and inclination of work sites.
Alat pemindah tanah harus dioperasikan sesuai dengan petunjuk pabriknya mengenai bahan, kecepatan,
putaran, dan kemiringan daerah kerja.

(5) It is prohibited to swing the dipper or the bucket above the cabin of any transport vehicle until the operator
has left the cabin and is in a safe place, except where the said vehicle is specifically designed to protect

Article 254 – Equipment
Pasal 254 - Alat

(6) It is prohibited to use earth moving equipment to lift or transport hazardous materials.

Dilarang menggunakan alat pemindah tanah untuk mengangkat atau mengangkut bahan berbahaya.

(7) Where earth moving equipment is moving to another location, the digging equipment or excavation bucket
shall be lifted just above the ground so as not to interfere with the operators view. In the case of draglines,
the bucket shall be kept as close as possible to the boom.
Apabila alat pemindah tanah sedang berpindah tempat, maka alat gali atau ember gali harus diangkat
sedikit dari tanah tetapi tidak mengganggu pangan operator dan untuk dragline, ember harus ditahan
sedekat mungkin ke lengan alat tersebut.
