We, CORAZON D. BANDICO, married, MYLYN D. RUGASAN, married,

SALVADOR EMMANUEL A. DE JESUS, single, LYN D. CAMERO, married,
RAYNETO A. DE JESUS, married, IMELDA D. BANCAIREN, married, all of legal age,
Filipino and residents of Balugo, Sibulan, Negros Oriental, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. We are the surviving legal heirs of TRANQUILINA ABLONG DE JESUS who died on
December 24, 2020 at Dumaguete City, and her deceased spouse LEON C. DE JESUS,
Jr. who died on July 31, 1996 at Hawaii, United States of America;

2. The following are our individual birth dates, as specifically stated in our Certificates of
Birth as attached herein:

a. Corazon D. Bandico - December 10, 1961;

b. Mylyn D. Rugasan - April 28, 1980;
c. Salvador Emmanuel A. De Jesus - December 25, 1966;
d. Lyn D. Camera - August 04, 1976;
e. Rayneto A. De Jesus - February 13, 1972;
f. Imelda D. Bancairen - May 13, 1969.

3. We are executing this Affidavit to attest to the truth of the foregoing, and for whatever
legal purposes this may serve best.
