RHSEI findings

November 2016
The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.

What happened?
Multiple H2S related safety findings were found during several Rig HSE
Inspections (RHSEI):
• Main Camp gas monitoring system at Mess Hall not working.
• H2S Panel at Camp Boss office showing error readings.
• H2S sensors incorrectly positioned and not protected from mud/water
splashes.
• H2S alarm in workshop not audible.
• One 15 minute SCBA missing at Mud Mix Area.
• Cascade system leaking.
Consequences:
• High potential for fatality in the event of a H2S emergency.
• H2S emergency response equipment unable to function as needed.
Why did it happened?
• Deviation from established safety equipment guidelines.
• H2S detection and protection systems not maintained per requirements.
• Delays in rectifying problems related to H2S detection and protection
systems.
• Rig management not giving high priority to H2S related safety findings.

What should be done to prevent recurrence?


• Conduct safety equipment checks per D&WO and contractors’ SMS
maintenance guidelines.
• Rig site visual and audible alarm system are repeated to the Main Camp
alarm system when the rig main camp is located within the 30 ppm HS2
RER.
• Prompt rectification of any issues related to H2S detection and protection
systems.
