AFFIDAVIT OF LOSS

I, GERHARD G. ALDENESE, of legal age, single, Filipino and a resident of

Lunzuran, Zamboanga City, after being duly sworn to in accordance with law, depose
and say THAT;

1. I am in possession and holder of a Sun Cellular Telecom Pre-Paid Sim Card under
Cellular Phone Number 0933-590-4439 of which I inserted in my phone;

2. That I misplaced my Sun Cellular Pre-Paid Sim, got lost and cannot be located and
after exerting diligent effort to look for the same, it cannot be located and is
deemed lost beyond recovery;

3. I am still intending to use the same Sun Cellular Telecom Pre-Paid Sim Card
under Cellular Phone Number 0933-590-4439 as it has been my cellular phone
number for the past years and most of my account binds with the number thereto;

4. That I am executing this affidavit to attest to the truth of the foregoing narration of
facts, more particularly to declare the loss of my Sun Cellular Telecom Pre-Paid
Sim Card under Cellular Phone Number 0933-590-4439 and to issue a new Sun
Cellular Pre-Paid Sim Card under the same number, 0933-590-4439, and for
whatever legal purpose this may serve.
