When would you go to an adult with concerns that

another student is suicidal or depressed?

Put the number on the line according to when you would go to an adult if you had a friend who:

1. eats a lot/eats too little 6. wants to be alone all the time

2. is experiencing a major stressful event 7. is having a bad week
3. talks about suicide or death 8. sleeps a lot/sleeps too little
4. tells you “I am going to kill myself” 9. gives you something valuable
5. cries a lot 10. tells you “I am going to kill myself but asks you to keep it a secret.
