Advisory: Temporary Closure of Intellicare Hubs
To ensure the safety of our frontline personnel following reports of the increasing number of new
COVID-19 cases, please be informed that effective March 22, 2021, all our hubs with assigned
Field Customer Service Specialists (FCSS) and corporate clinic nurses will be closed until further
notice.
Members availing outpatient consultation and diagnostics have the following options to request
for an electronic referral control sheet (ERCS):
1. Download the Intellicare Agora mobile app which is available at the App Store and
Google Play.

2. Send an email to referralcontrolsheet@intellicare.ph. We will cater to both internal and
external clients from Mondays to Saturdays, 8:00 am to 5:00 pm. For easier facilitation,
we would be needing the following details.

Contact Number:
Soft copy of your Intellicare card and 1 valid ID with picture
3. For ERCS requests beyond the regular hours indicated above and for other facilities not
indicated in the list, please email css@intellicare.com.ph.
4. Print two (2) copies of the issued electronic RCS and sign the form before going to the
accredited doctor for consultation and/or hospital department for diagnostic procedure.
