Joint Affidavit
We, EDELESA DANGIN BARTULO and VIRGELIO MAHINAY JARIOLNE, of legal
ages, Filipino Citizens, and residents of Iligan City, after having duly sworn to an oath as
required by law, hereby depose and say:

1. That we personally know the Late, HELEN JARIOLNE MANAYTAY born to Eduardo
V. Manatay Sr., and Elisa M. Jariolne on October 1, 1974;

2. That the Late, HELEN JARIOLNE MANAYTAY died on December 21, 2020 in Iligan
City as evidence by hereto attached Certificate of Death;

3. That the Late, HELEN JARIOLNE MANAYTAY, has a child in the name of Lovely
Ann Manatay; however, the Late HELEN JARIOLNE MANAYTAY died single and
unmarried, hence in the Certificate of Death, appearing therein the “SINGLE” marital
status of the latter;

4. That we are executing this affidavit to establish the true and correct marital status of
the late, HELEN JARIOLNE MANAYTAY and for such other legal purposes it may
serve best.
