to help you with mortgage and/or utility

payments if your income has been
negatively impacted by the COVID-19 DOCUMENT CHECKLIST: The following supporting items are
pandemic. You may be eligible for up to six required before your application can be approved for
months consecutive emergency assistance:
Completed application
mortgage/utility assistance.
 Completed and signed (ShelterNet) Release of Information
No direct payment assistance to  Self-Declaration of Income Form
individuals.  DOCUMENT ALL INCOME
Types of Income Documentation
Payments will be made by Charleston o Paystubs from April 1, 2020 to the present date from
County directly to the mortgage holder employers for each working member of the household
o Current Social Security and/or Social Security Disability
and/or utility company.

pandemic (Example: termination or reduction in hours letter
from employer due to pandemic, unemployment, limited
resources, increased expenses, etc.)
IMPORTANT:  Most recent mortgage statement
 Signed letter (including mortgage account number and property

License, Government ID, Current Passport, etc.)
