1. INTRODUCTION

The PlatinumGMT™ Diode Laser System device is indicated for use in long-term
photoepilation treatments.

PlatinumGMT™ Diode Laser System features all of the technological advances


needed to perform safe, efficient, fast and very comfortable treatments. The
machine is designed for the long-term and progressive reduction of unwanted body
hair.

PlatinumGMT™ Diode Laser System works based on the selective


photothermolysis principle, in which parameters are adjusted to selectively
eliminate hair follicles without affecting the adjacent tissue.

PlatinumGMT™ Diode Laser System consists of two main parts: the central unit
with the control cover with touch screen, and the hand unit, which contains the
luminance unit (laser).

The PlatinumGMT™ Diode Laser System operator can choose from three modes
of operation, Standard, Fast (sweep) and Single, which are set directly on the
screen.

The parameters: power level (fluence), pulse width (pulse), burst frequency
(frequency) and spot cooling (cooling), based on the specific characteristics of
each client’s hair and skin. The energy level parameter defines how much energy
is delivered to the site to be treated.

2. PRECAUTIONS

Users should carefully read this Manual, its precautions and protocols before
working with the PlatinumGMT™ Diode Laser System. This manual provides a
guide for performing the treatments, and is subject to improvements, changes or
variations. The client is responsible for damage of any type that may result from
the improper or faulty use of the equipment or from failing to follow the procedures
described in the classes or hands-on training received.

PlatinumGMT™ Diode Laser System is an electro-optical device designed to offer


safe and reliable treatments. Users of this equipment must strictly observe the
security precautions given in this section.

PlatinumGMT™ Diode Laser System is not designed to be used by people


(including children) with reduced physical, sensory or mental abilities, or who lack
experience or knowledge, unless they are supervised or have received training on
its proper use.

Children should be supervised to ensure that they do not play with the machine.

PlatinumGMT™ Diode Laser System is designed to maximize the safety of those
receiving (clients or patients) and applying (operators) the treatment. Any area of
the body can be treated, subject to the following precautions:

Use only on areas that DO NOT exhibit the following characteristics:


• Wounds, injuries, redness, edemas, erythemas, crusts, burns, irritation,
dermatitis or any change in the normal skin.
• Natural or artificial tanning agents of any kind.
• Hair removal using waxing methods.
• Tattoos, moles, dark spots or any hyper-pigmented skin.

IMPORTANT
Before starting any treatment, carefully read the list of contraindications
and the exclusion criteria.

3. WORKING PRINCIPLE
Principle of Selective Photothermolysis
When applied to the skin, the PlatinumGMT™ Diode Laser System diode laser is
absorbed by the chromophores in the treated area. The light energy absorbed by
the chromophores (hair melanin) is converted into heat energy, which acts to
coagulate cells.

Working Mechanism:
Diode laser works by using light to produce an increase in the hair’s temperature.
The light, which is absorbed by the hair’s melanin and not by the skin (to be
precise, to a lesser extent by the skin, which is lighter than the hair), heats the hair
to 70 / 80° C, carbonizing and coagulating its root. The thermal relaxation time
allows the skin to constantly reduce any excess temperature, thus avoiding
damage.

The blood that flows through the dermis allows the skin to be cooled. Hair,
however, burns. The intensity used will depend on the skin type and on the color
and thickness of the hair.

4. SYSTEM SAFETY

Though the PlatinumGMT™ Diode Laser System is a safe mechanism for the
eyes, clients must wear protective glasses during facial treatments to shield them
against any eye damage.

Only personnel authorized by Global Medical Technology should perform any work
inside the PlatinumGMT™ Diode Laser System protective covers. Inside, the
system has dangerous electric voltages and currents.

Ensure the proper voltage is supplied to the PlatinumGMT™ Diode Laser System
(see the label at the base of the central unit).

The operator’s routine maintenance should be performed at least 10 minutes after
the system is shut down and disconnected from the power main. Carrying out the
maintenance procedures with the system plugged in or within 10 minutes of
disconnecting it can be dangerous to the operator and can damage the system.

Electrical and mechanical safety


• Do not open or remove any covers on the PlatinumGMT™ Diode Laser System.
Removing any covers poses a safety hazard.
• PlatinumGMT™ Diode Laser System produces high voltages, which can be
dangerous. Some components can hold a charge once the power is off. Only
personnel authorized by Global Medical Technology should remove the covers.
• PlatinumGMT™ Diode Laser System is connected to the ground through the
connection in the power cord. This connection is essential for safe operation. Be
sure the electric output at your site is properly connected to the ground.

Fire danger
• The photothermic energy absorption increases the temperature of the absorbent
material. You should take preventive measures to reduce the risk of flammable
materials in the place of treatment.
• Do not use flammable substances, like alcohol or acetone, when preparing the
skin for treatment.
• Do not use flammable substances to clean or disinfect any part of the
PlatinumGMT™
Diode Laser System, mainly the spot (glass on the hand unit).

WARNING
 Sending excessive energy to the treatment site can cause thermal damage to
the skin, crusts, redness of the skin, hypo or hyperpigmentation.
 Never look directly into the main lens, as this can injure the eye.
 Never turn the machine on without first placing the hand unit.
 Never aim the hand unit to emit a pulse of light into space. Be sure to aim it
only at the treatment site.
 All light-emitting device pulses can cause injuries if not used correctly.
 There are high voltages inside the PlatinumGMT™ Diodo Laser System Cloud.
 Personnel who work with pulsed light sources should always be aware of the
possible danger and take the corresponding precautions, as described in this
User Manual.
 Freckles must be covered with white tape. Do not use flammable material.

Labels
The following labels are attached to the rear of the PlatinumGMT™ Diode Laser
System. The owner of the equipment should be familiar with the meaning and
location of these labels and check them regularly.

Characteristics of the treatment room


The room or cabin in which the treatments will take place with the PlatinumGMT™
Diode Laser System must comply with the following requirements:

a) As required by national regulations, the doors of the treatment room must be


clearly identified with a sign. This sign (in English or Spanish) must be as shown
below:

b) There should be 60 centimeters between the machine and the closest wall. The
dimensions of the machine are:
Wide: 35 cm
Long: 48 cm
Height: 115 cm

c) The machine should be far away from any heat sources.

d) Electrical requirements: 110-120V 50/60Hz 12A.

e) The presence of corrosive materials in the work area can damage the electrical
system of the machine and its optical components.

f) The presence of a high dust concentration and suspended particles in the work
area can damage the machine by getting inside the covers. It can also absorb a
portion of light emitted by the system.

g) The presence of metallic dust in the environment can damage the machine. The
PlatinumGMT™ Diode Laser System should be used in a non-corrosive
atmosphere.

h) Temperature of work area: The work room should be kept at a constant
temperature between 20 and 25º C and a relative humidity under 80%.

i) The work area should be air conditioned.

Optical Safety Requirements


The following recommendations should be followed meticulously to avoid harmful
effects to the eyesight of both operators and patients.

• Be careful with accidental exposure to the light emitted by the machine. Everyone
present in the treatment area should use special protective glasses, provided by
Global Medical Technology.
• Clearly identify the work room so as to avoid having anyone enter unprotected.
• Only those whose presence is required should be in the treatment room.
• Anyone involved in providing treatment must be properly trained on and informed
about the use of the equipment.
• The equipment can only be used by qualified personnel.
• Never look directly at the light emitted by the machine. Never look directly at the
lens through which the light is emitted when the machine is on. Never look at the
reflective surfaces during a treatment.
• Never aim the hand unit at any surface or object that is not specified in this user
manual.
• Not work with the machine close to any flammable or explosive substances.
• Never allow the presence of reflecting surfaces, like mirrors, glasses, watches,
jewelry and surgical instruments, in the treatment area.

Power key
The power key on the machine connects and disconnects the power to the system.
The machine works only with the power key supplied by Global Medical
Technology.

IMPORTANT
Never leave the key in the machine unsupervised.

Ensure the system is not on when you remove the power key.

Safety systems on the machine


Emergency circuit breaker (Stop)
One automatic breaker (Red Stop Button) located at the front part of the machine
controls the electricity consumption of the main unit, automatically interrupting the
power in case of an accident, overcurrent or any irregularities in the electrical
supply.

Auto Test
When the machine is turned on, a self-test checks the electrical system and the
network connection. This process is carried out every time the hand unit is
changed, to avoid overload or the existence of remaining amounts of energy that
may alter the amount of energy delivered with every pulse.

System Installation
Follow these steps to install the PlatinumGMT™ Diode Laser System:
• Unpack the machine and place it in an appropriate place.
• Check the machine’s integrity and its components
• Fill the tank with distilled water.
• Connect one hand unit.
• Plug the machine into an electrical outlet.
• Switch on the machine by turning the power key. Ensure that the control screen
turns on correctly.

IMPORTANT
Never turn on the machine without connecting the hand unit first.
Never use the machine with a low water level.

Moving the machine


To move the machine from one place to another, observe the following
requirements:

1) Unplug the power cord from the outlet.


2) Disconnect the hand unit.
3) Lift the brake wheels.
4) Push the machine slowly on its wheels.
5) The machine should not be put on its side or in any position that is not
vertical.
6) If it must be moved horizontally, put the stopper in the fill valve and verify the
water level immediately after being moved.

System Components
The equipment consists of a control panel with a touch screen (user interface) to
monitor the parameters, an electronic system that controls the power supply based
on the pulse type selected, and a cooling system with a water pump, a 4-liter tank
and a radiator, which allow for spot cooling of the handpiece.

Hand unit
The hand unit is the part of the machine that contains the light emission system
(diode laser). It is connected to the main unit by a power cable and hoses through
which the spot cooling water flows.

The lens that shapes the filtered light protrudes from the surface of the head. The
light is activated when the push button on the handle of the hand unit is pressed.
The area covered by each shot is about 10 mm x 12 mm (120 mm²).

Normal use of the unit


1. Do a visual inspection to check the water level.
2. Connect the handpiece to the unit.
3. Plug the unit to the outlet and turn the key to turn it on.
4. Touch the screen with a finger and select the treatment mode to be used: Fast,
Standard or Single.
5. Press the keys to adjust the power in Fast mode.
6. In Standard mode you must first select the desired parameter and then the
keys to adjust it.
7. When sure of the parameters to use, press "ON".
8. Next, place the handpiece over the area to be treated and press the button to
activate the laser.

Control panel
The control panel consists of an LCD touch screen and the key and emergency
switches.

Each option can be modified by touching the screen directly with the finger.
Each of the screens is described below:
1- TREATMENTS screen: Accesses the different treatment modes and the
information on each:
- FAST MODE: Quickly sweeps over the treatment area. Normally used
for thick hair.
- STANDARD MODE: Single pulses over the treatment area. Normally
used for thin hair.
- SINGLE MODE: Single pulses over the treatment area, with each pulse
emitted when the button is pressed. Normally used for thin hair.
2- Selecting FAST MODE opens the SKIN TYPE screen, where you can
select:
- HIGH or LOW DENSITY (of hair)
- LIGHT OR TAN
- PHOTOTYPE (I, II, III, IV or V)

3- TREATMENT AREA SELECTION. The area to be treated is selected, and
the client’s code can be entered.
4- WORKING PARAMETERS screen in FAST MODE. The parameters are
pre-set, based on the previous selections, but some of them can be
modified.
- ENERGY ACCUMULATED: Adjustable parameter. This is a countdown
timer of the energy (in KJ) still to be applied to the area during the
treatment.
- TIME PER AREA: A countdown timer of the treatment time remaining.
Parameter calculated based on the settings for: ENERGY
ACCUMULATED, FREQUENCY AND PULSE INTENSITY.
- RESET AREA button: Can be pressed to reset the total once the TIME
PER AREA runs out.
- FREQUENCY: Pulses emitted per second (1 to 10 Hz). Adjustable
parameter.
- COOLING: Level of spot cooling of the handpiece (1 to 5). Adjustable
parameter.
- PHOTOTYPE: Parameter selected on the SKIN TYPE screen.
- AVERAGE POWER: Parameter calculated based on the FREQUENCY
and PULSE INTENSITY set, and which varies between 1 and 10 J/cm2.
This is the energy accumulated in Joules/cm2 in a tenth of a second.
- PULSE INTENSITY: Adjustable parameter. The power of each pulse
emitted. It is the energy of a pulse in Joules/cm2, range is from 5 to 10
J/cm2.
- TIMER: Adjustable parameter. A countdown timer of the treatment time
remaining. It only counts when the unit is ON.
- SESSION COUNTER: Shows the session minute and pulse counters. It
only counts when the unit is ON.

- PULSE: The pulse width in milliseconds.
- FREQUENCY: Pulses emitted in one second (Hz).
- POWER: Pulse energy in J/cm2.

6. TREATMENTS
The instructions below must be followed when using PlatinumGMT™ Diode Laser
System to safely remove hair. This information is based on the observations of
clinicians experienced in the use of this technology.

This document is intended as a guide. It is not a substitute for professional training,


certification, and supervised practice. For correct use, follow the instructions of this
user manual.

The use of a large amount of energy or of repeated or excessive pulses can cause
overtreatment, leading to undesirable results, like irritation, pain (similar to
sunburn), itching, redness of the skin and hyperpigmentation.

Careful monitoring and a conservative treatment will avoid these problems.

Therapeutic indications
The PlatinumGMT™ Diode Laser System can be used for treating different parts of
the body in patients with skin type I, II, III or IV with particular caution, on the
Fitzpatrick skin chart.

Exclusion Criteria for hair removal


The following MUST BE NOT TREATED:

• Persons under the age of 18.


• Pregnant and lactating women.
• Recently waxed áreas (last 6 weeks).
• History of keloid formations or other skin diseases.
• Open wounds.
• Hair from tattooed areas.
• Eyebrow hair.
• Areas exposed to the UV-radiaton (sun, tanning bed) in the last 30 days.
• Clients who have taken isotretinoin (Accutane) in the last 6 months.
• Clients who have taken photosensitive medications in the last 4 weeks.
• Use of anticoagulants.
• Clients with epilepsy.
• Clients who are allergic to sunlight, such as those with porphyria, lupus, solar
urticaria, etc.
• AIDS or any other disease that weakens the immune system.
• Skin or other cancers.

Pre-treatment considerations
During the first visit, the areas to be treated must be evaluated. The following steps
are also required:

1. Medical Record – Fill out the client card, including previous treatment
methods
2. Determine if the client is suited to treatment with PlatinumGMT™ Diode
Laser System.
3. Explain to the client what treatment with PlatinumGMT™ Diode Laser
System entails.
4. Explain to the client:
• The hair growth cycle for different parts of the body. This will help clients
understand the necessary treatment program.
• That they may experience discomfort during the treatment.
• That erythema, edema or temporary blistering is possible immediately
after the treatment.
• That there is a risk of side effects, like hypo or hyperpigmentation of the
skin. These are generally temporary.
5. LOCAL ANESTHESIA – The use of a local anesthetic cream (ex. EMLA) is
allowed to relieve any discomfort.
Normally a local anesthetic is applied an hour before the treatment begins.
The operator must make sure to remove all the anesthetic product from the
skin before commencing the treatment.
In this case the parameter settings (frequency, Joules) must be monitored
closely because the skin loses its usual sensitivity and it can be damaged
by the heat. Use of conductive gel or cold gel strips is always
recommended.
6. Before the treatment, the client should sign the informed consent form (ICF).
7. EVALUATION - Evaluate the client skin type and decide if it is sensitive to
the sun. Does the skin in injured areas heal in a darker color? This can
reveal potential pigmentation changes.
8. DETERMINE THE SENSITIVITY OF THE SKIN – Is the patient’s skin easily
irritated while shaving? Did he/she have a reaction to wax hair removal? Is
the skin sensitive to creams or lotions?
9. Do a test by applying 2-3 shots, as shown below.

Pre-treatment preparation
1. Clean – Remove any perfume, make-up or cream residue from the skin with
a towelette and dry the area thoroughly.
2. Shave – Shave the area completely before the treatment. This should be
done 24 hours earlier to avoid irritating the skin further after shaving.
3. Moles – Moles should be covered with a white pencil.
4. Conducting gel – Place a thin film of conducting gel on the skin to ensure
the handpiece makes perfect contact over the area to be treated.

Procedure
1. Evaluate the client’s skin type as per the aforementioned Fitzpatrick chart.
2. Select the test location.
3. Once the skin is prepared, apply two or three pulses to the test area at two or
three different energy levels, taking into account the client’s skin type.
4. Wait about 10 minutes to see the clinical response to the treatment. Select an
energy level at which changes to the skin and follicles are visible with no side
effects.
5. Check the response of the skin after firing a test pulse.
6. If everything is ok, continue the treatment with the same working parameters.
7. Check the skin’s reaction to the treatment periodically, watching for changes
in skin color or for erythema, edema, etc. There may also be an odor of
burned hair during the treatment. The absence of this odor, however, does not
mean that the parameters are incorrect.
8. If there are any adverse reactions in the skin, adjust the parameters lower to
reduce the amount of energy being delivered to the skin. This can be done in
one of two ways: by lowering the power or by changing the pulse width
(making it wider).
9. If no adverse effects are noted and the skin retains its normal look, you can
increase the energy level one step if necessary.
10. Once the treatment is complete, proceed immediately to the post-care
treatments specified below in this user’s manual.
11. Turn the unit off if it will not be used for an extended period of time.
TREATMENT OF FACIAL AREAS
The PlatinumGMT™ Diode Laser System must not be used to treat areas
around the eyes. Areas near the eyes must be treated with extreme care, and
only those areas:
• From the eyebrows up.
• Leaving two centimeters from the corners of the eye.
• From the cheekbone down.
12. CLEANING AND MAINTENANCE OF THE HAND UNIT - After each
treatment with PlatinumGMT™ Diode Laser System, clean the unit with a
cloth (or paper towel) and then disinfect the head on the handpiece with
cotton moistened in alcohol. Keep moisture from entering the lamp cover.
Check the integrity of the lamp and the protective screen before each
treatment with PlatinumGMT™ Diode Laser System.
13. ADJUSTMENTS FOR THICK, DENSE HAIR: reduce the energy for the
initial treatment until the hair regrows thinner and less dense. Increase the
energy in subsequent treatments if necessary.
14. ADJUSTMENTS FOR FINE HAIR: If the energy level is working for most of
the hair but not on those areas where the hairs are thinner, increase the
energy level.

Some clients might want to be treated again earlier, but the PlatinumGMT™ Diode
Laser System is most effective when the hair is at least 1 mm long.
After some treatments there should be a greater interval between hair growth
cycles, depending of the amount of hair and on the client’s particular growth cycle.

Post-treatment
Cold (but NOT frozen) bags can be used after the treatment to prevent burning and
to make the patient feel more comfortable. A soothing gel or a cloth soaked with a
soothing agent can be applied to the treated area.
The proper steps must be taken to keep the area from getting irritated in the four or
five days following the treatment. This means avoiding hot baths, aerobic exercise,
massages, etc.
• Patients must apply a sunscreen with a high protection factor (SPF>30) and
avoid exposing the treated area to the sun for at least one month after the
treatment. If the treated area is exposed to the sun, hyperpigmentation can
result due to the over production of melanin.
• Typically, a minor edema lasting one day or a minor erythema lasting 1 to 3
days should be monitored. If the side effects last longer than this, the client
should see a doctor.
• Do not apply make up until 24 hours after the treatment. Do not use if the
skin develops a burn or a scab. Since the skin will be very sensitive after the
treatment, special care should be taken when removing make up, as it can
infect the area.

Treatment follow-up
The recommended follow-up for the treatment is given below.
Under normal conditions, the patient should return every 6-8 weeks for facial
hair and 8-10 weeks for body hair so the specialist can evaluate the area and
determine if another hair removal session can be conducted. This time can vary
depending on each patient’s unique hair growth cycle, as well as on the previous
session’s results.
The hair may change in appearance, for example from thick to thin. This
should be considered when setting the treatment parameters for subsequent
sessions.
After the second treatment session, the time between sessions can be
prolonged to allow the hair to grow in the area to be treated. This will depend on
the area of the body and on the patient’s individual characteristics (man/woman,
hormonal problems, etc.).

Completion of the treatment


The treatment is complete when the patient is satisfied with the results or when
deemed appropriate by the specialist.

7. MACHINE MAINTENANCE
The safe and effective use of the PlatinumGMT™ Diode Laser System requires
proper care in order to keep the machine in good working condition.

Cleaning the main unit: The machine must be clean at all times. Before cleaning
the main unit or the hand unit, unplug the machine from the outlet.

Do not use flammable and abrasive substances. Use a soft cloth and specific
cleaning solutions for plastic.

The machine must be cleaned periodicaly, primarily the grooved parts where the
air that cools the water circulates, particularly the ones on the bottom part of the
device.

Cleaning the hand unit: The hand unit should always be cleaned after use. For
reasons of hygiene, never use on a patient without clearing the main lens.

Use a soft, moistened cloth that does not leave fibers behind. Wipe the entire
surface of the lens with it.

Never submerge the hand unit in liquids. If liquid contacts the unit’s internal
components, immediately call the Global Medical Technology technical
service on (+34) 902 02 41 91.

Changing the distilled water: The machine may overheat if the water level is low.
Check the water level daily. Every 1-2 months of continuous operation the water
must be replaced.

Use distilled or deionized water only. Any other kind of water will result in the
formation of calcareous plates inside the system, causing it to break.

To change the water:


a) Insert the short hose in the drain valve (Drain) until the tank is
completely empty.
b) Insert the short hose in the OVERFLOW valve to let the air out while
the tank is filled with water.
c) Connect the fill hose between the bottle and the WATER INLET valve.
d) Fill with water until the transparent indicator exceeds the minimum level
indicated, or better yet, until water issues from the OVERFLOW valve.
e) Remove the hoses from the valves.

APPENDIX

On the use of the hand units and lenses of the PlatinumGMT™ Diode Laser
System:

1. Replace the hand unit when the machine is off.

2. Store the hand unit in a safe place. Warning! The components can be damaged
if struck. If you see anything abnormal, please stop the machine and do not use
the hand unit.

3. Clean the outside of the light guide with a soft cloth. Warning! Clean the glass
on the light guide after using it.

4. Be sure to use the correct gel to obtain a safe and efficient treatment. Warning!
Improper use of the hand unit or using an inappropriate gel may damage the light
guide on the lens. It could also cause bad treatment results, such as redness and
slight damage to the skin.

5. The handpiece is capable of producing millions of pulses and thus has a very
long lifetime.

6. If you see anything abnormal in the glass or in the rest of the handpiece, or if it
does not work properly, contact the technical service department of Global Medical
Technology.
