AFFIDAVIT OF LOSS TITLE

I, AMABLE M. URIETA, JR, Filipino, of legal age, married and

with residence address at Barangay Poblacion, Sablayan, Province
of Occidental Mindoro, represented by ROLANDO B. URIETA,
Filipino, of legal age, married and with residence address at
Barangay Poblacion, Sablayan, Province of Occidental Mindoro, via
SPECIAL POWER OF ATTORNEY, after being sworn in accordance
with law, do hereby depose and state:

1. I am the purchaser/vendee of that parcel of land located at

Barangay Poblacion, Sablayan, Occidental Mindoro, containing area
of SIX HUNDRED FORTY SIX (646) SQUARE METERS covered by
Original Certificate of Title No. P-9120 of the Registry of Deeds of
Mamburao- as evidenced by Extrajudicial Settlement of the
Estate of the Deceased Loreto V. Urieta and Presentacion
Bundang with Sale and Waiver of Rights which was executed in
my favour by AMABLE B. URIETA, SR., ELMER B. URIETA,
VICTORIA URIETA LIEN, EUNECIE URIETA AMPONIN, FELIPE
B. URIETA, EFREN B. URIETA, MEMORY URIETA GONZALES
AND ROLANDO B. URIETA, and thereupon acknowledge before
Notary Public, ATTY. MELVIE E. SILVERIO, and docketed in her
Notarial Registry as Doc. No.3624; Page No. 96; Book No. VII;
Series of 2018. A copy of said Extrajudicial Settlement of the Estate
of the Deceased Loreto V. Urieta and Presentacion Bundang with
Sale and Waiver of Rights is hereto attached as ANNEX A;

2. Immediately after the execution of the said Extrajudicial

Settlement of the Estate of the Deceased Loreto V. Urieta and
Presentacion Bundang with Sale and Waiver of Rights, the
original owner’s duplicate copy of OCT No. P-9120 was turned over
to me by AMABLE B. URIETA, SR. He likewise delivered to me the
possession of the subject parcel of land and since then I have
openly exercised and maintained all rights of ownership thereon;
 3. I was not able to immediately transfer the title in my name
because I was then preoccupied with heavy workload and other
personal concerns. Sometime in August 30, 2018, when I decided
to process the transfer of title in my name, because I finally had the
time and opportunity to do so, I discovered that the original
duplicate owner’s copy of Original Certificate Title No. P-9120 was
missing; I could no longer locate it on my files. All diligent efforts I
exerted to locate and retrieve it were in vain; hence for all legal
intents and purposes it is now deemed lost and beyond recovery;

4. I certify that the parcel of land covered by OCT No. P-2160

of the Registry of Deeds of Mamburao was never mortgaged, sold,
conveyed, disposed of, or encumbered, to any other person or
entity. I certify and confirm further that said document was never
confiscated nor relinquish in favour of the government entity,
agency or authority;

5. I attest to the truth of the foregoing statements to the best

of my own personal knowledge and based on authentic documents
or records. I am executing this Affidavit of Loss to support my
application for a new owner’s duplicate copy of OCT No. P-2160 as
well as for all lawful intents and purposes it may serve.
