Turning and Repositioning a Client on Bed

Procedure
Assessment
1. Assess client’s physical abilities, level of comfort and ability to follow instructions.
To provide information about client’s physical status and number of people needed for safe turning
and position.
2. Check client’s chart for diagnosis.
Because there are specific restriction in positioning client

Planning
3. Wash hands.
4. Organize and check the device for its function. To avoid accidents while transferring client

Implementation
5. Identify the client. Explain the procedure to the client. To promote cooperation and enhance understanding
of procedure
6. Raise bed to high position. To prevent muscle strain in care provider
7. Put bed in a flat position. To prevent working against gravity
8. Secure all tubes, drains, and IV’s. To prevent accidental dislodgement of tubes
9. Place or assist client in a starting position. To prevent client from injury

Moving Client from Supine to Side Lying Position


10. Have the client flex his knees and place his arms across his chest. To prevent client from rolling back
11. Place your hands on the client’s shoulders and hip on the far side. To role client’s heaviest part
12. Spread your feet, flex your knees and place one foot behind the other, while rocking backward, gently roll
the client towards you. To provide a wide base support
13. Raise the bed side rails and move to the opposite side. To prevent client from rolling out of bed
14. Adjust the client until he/she is in correct side lying position. To help client maintain the position
comfortably
15. A pillow may be placed to support client to head and arms. To provide support

16. Wash hands.


17. Evaluate client’s response to the procedure, position, and any abnormal findings.
18. Record the procedure done To provide accurate data on the patient care
