Roles and Responsibilities within


the Permit to Work System

For some work activities personnel may be


responsible for covering more than one role.
• Operating Authority
• Permit Controller
• Permit Applicant
• Worksite Supervisor
• Permit User (Work Party Member)
•Radiation Protection Supervisor
• Classified Person (Radiography)
• Authorized Gas Tester,

Operating Authority

Permit Controller

I control and co-ordinate the issue and closure of


Work Permits.

I am the Supervisor of a Station / Plant / Facility

I have an overview of all operational activities to


avoid potential risks of simultaneous activities.
I will detail the potential hazards
involved in the work activity in the appropriate box in
the Permit.

Performing Authority
I am the representative of the sponsoring department / company

responsible for initiating the work appointed by the Department Head.

I initiate and raise the Work Permits and any supporting Certificates.

Needs the technical competence to consider and guard against potential


hazards associated with the work activity

I am responsible for appointing Worksite Supervisors and ensuring that


all personnel under my authority are competent.

Work parties receive necessary training & instructions relevant to QP or


special precautions required for the commencement of work

Nominated fire watch & safety watch are approved by HSE Dept.

Work Supervisor

the toolbox talk and specified on the Work Permit.

Radiation Protection Officer


(RPO)

• Understand the necessary precautions to


be taken for the work to be performed &
ensure precautions restrict exposure

• I am responsible for initiating all


radiography work.

• It is also responsible to monitoring the


work of the radiation worker

Radiation Workers

¾ designated to be associated with industrial


radiography.
¾All personnel engaged in radiography work shall
wear a personal monitoring badge (RIIM Badge) and
carry a personal radiation monitor (Dosimeter) in
areas where the radiation dosage may exceed 7.5
micro Sieverts per hour (equivalent to 0.75 millirems
per hour).

Authorized Gas Tester

¾Gas testing equipment has been calibrated and is in reliable


working condition prior to use.
¾I am responsible for carrying out toxic, flammable
and oxygen gas tests in support of Work Permits
¾ Appointed by operating authority
¾Ensure gas testing equipment has been calibrated
¾Stop the work incase of abnormal gas test result & inform to
permit controller & work supervisor

Firewatcher

• I will monitor the worksite during a Hot Work Permit


activity or required by permit controller during hot
work

• I am appointed by the Performing Authority and


accepted by the Fire and Safety Department.

• I have basic fire fighting knowledge so I am


competent in the use of fire hoses as well as the
portable equipment.

• I am capable of interpreting the portable gas


monitors and understand what to do in the event of
an emergency and raise the alarm.

Fire Watcher shall ensure that:

• All flammable materials are removed from the worksite.

• Drains remain covered and water sealed.

• Sparks / welding spatter are contained by use of fire blankets, water spray

• Fire fighting equipment is available and ready for immediate use.

• Continuous gas detection equipment onsite is closely monitored.

• An alarm is raised if fire or gas emission occurs in the worksite area.

General Responsibilities:

All personnel within the Permit to Work System also have general
responsibilities.
These include:
• Following agreed work conditions
• Ensuring the specified precautions are taken
• Being aware of other work in the same area or system
• Knowing the location of the nearest fire extinguishers, breathing
apparatus (BA) equipment, communication equipment and
manual alarm call points

Emergency Alarms

• When alarm sounds, other than for announced test, all


work is suspended.

• You should make the worksite safe, as far as


practicable, without risk to personal safety.

• All personnel should then proceed to their assembly


point.

• All Work Permits must be re-endorsed before work can


re-commence.
