MEMORANDUM OF AGREEMENT (MOA)

between
JPG PRINTING SERVICES
and
Eastern Star Academy, Inc.
for
ID Card Production for ESA’s Students and Employees

I. PARTIES
The parties to this MOA are the CONTRACTING AGENCY, hereafter referenced as
" JPG PRINTING SERVICES" and the CUSTOMER AGENCY referenced as
“Eastern Star Academy, Inc”.

II. PURPOSE
The purpose of this Memorandum of Agreement (MOA) is to specifically identify the
scope of services to be provided, establish the authorized individual or agency as
the decision-maker or owner in the agreement, as well as the financial and human
resources that are required for the provision of services. This MOA will also clearly
identify the roles and responsibilities of each party as they relate to providing the
consolidated DESCRIPTION OF SERVICES that serve both JPG PRINTING
SERVICES and Eastern Star Academy, Inc.

III. LINE OF AUTHORITY

1. JPG PRINTING SERVICES and Eastern Star Academy, Inc are legislatively
established as separate agencies with distinct appropriations.
3. The relationship between JPG PRINTING SERVICES and Eastern Star
Academy, Inc as defined by this MOA is one of contractor to customer.
4. INDIVIDUAL OR POSITION at JPG PRINTING SERVICES and INDIVIDUAL OR
POSITION at Eastern Star Academy, Inc will be the Contract Administrators for
this MOA.

IV. CONTRACTOR RESPONSIBILITIES

1. JPG PRINTING SERVICES ensures quality of productions for the ID Card.
2. JPG PRINTING SERVICES shall communicate properly to the school.
3. JPG PRINTING SERVICES shall complete the productions in due time.

V. CUSTOMER RESPONSIBILITIES
CUSTOMER AGENCY shall undertake the following activities during the duration of
the MOA term:
1. Ensure adherence of Eastern Star Academy, Inc to pay for initial payment for
the ID Card Productions.
 2. Eastern Star Academy, Inc shall pay the remaining balance, once the initial
outputs for the GS and JHS students which is due until October 31, 2020.
3. Ensure adherence of Eastern Star Academy, Inc to pay on time, and never
delay, to enable the production of the ID card for the students and employees of
ESA.
4. Eastern Star Academy, Inc will provide essential personal data of 725
students, including the 20 other employees.

VI. EFFECTIVE DATE AND SIGNATURE

This MOA shall be effective upon the signature of authorized officials of JPG
PRINTING SERVICES and Eastern Star Academy, Inc. The production
shall be completed at the end of the month of November, 2020.
