E. Participant’s The participants were the Department Head and teachers in the said
Description department.

F. Activity In line with the implementation of Republic Act No. 10533, or the
Description / Enhanced Basic Education Act of 2013, the Department of Education (DepEd)
Rationale issues the enclosed policy on The Learning Action Cell (LAC) as a K to 12
Basic Education Program School-Based Continuing Professional
Development Strategy for the Improvement of Teaching and Learning.
Through this policy, the DepEd fully supports the continuing professional
development of its teaching personnel based on the principle of lifelong
learning and DepEd’s commitment to the development of teachers’ potential
aimed towards their success in the profession. 
With this, the MAPEH Department hold its second SLAC of the quarter
to ensure that this policy is duly observed.
G. Objectives The activity aims to:

1. Discuss Quarterly Performance Assessment Templates in


preparation for SMEA.
- The needed data
2. Participate on the Search for the Most Outstanding Teacher.
3. Orient teachers to the forms needed in monitoring learners.

A. Participant’s The following were provided as evaluative observation of the participants:

1. Design of the Event
SLAC session in the small group good for two hours.
2. Methodology
- The resource speakers were knowledgeable to their assigned topics and
the teachers were actively participated the discussion too.
3. Management of the Activity
The objective of the above-mentioned activity was realized and carried out
thru the cooperation, effort, and observance of all the persons involved.
4. Resource Persons
Resource Speakers and teachers in the Mathematics Department
5. Audience Participation
The teachers were actively participated the discussion and submitted their
output at the end of the session.
6. Logistical Arrangement
All expenses relative to the conduct of this activity was chargeable against
School MOOE/Local Funds.
