AFFIDAVIT

I, ELNA R. SUBONG, of legal age, Filipino, and a resident of 20 St.,

Westwood Subdivision, Dungon C, Manduriiao, Iloilo City, Philippines,
after having been duly sworn to in accordance with law hereby voluntarily
depose and say that:

1. I am a the mother of JAIRUS EMILIO R. SUBONG, 12 years of age, and

is currently under my custody;
2. I was the one who procured the application of my son in obtaining a
Philippine Passport before the Department of Foreign Affairs (DFA),
Iloilo;
3. Pursuant to the requirements set forth by said department, an
Identification Card is necessary;
4. However, after graduating from Grade 6 at Luis A. Mirasol Memorial
Elementary School, Dungon B, Jaro, Iloilo City, I decided to not enroll
him for the succeeding school year due to COVID-19 precautions;
5. Such was the reason why my son has no valid Identification Card as of
the moment;
6. I am executing this affidavit to the truthfulness of the foregoing. This
Affidavit attests for the purpose of allowing my application for
Philippine Passport of my son, to be processed and approved in lieu of
an Identification Card and for any legal purpose(s) it may serve.
