 Affidavit of Self-Adjudication

Leonora Joaquin was devastated when her parents both died in an accident. After the
interment, she looked for all the properties left by her parents and she found out the following:
Insurance Policies with an amount of Php100,000.00 each; monies in banks in the total amount of
Php150,000.00; house and lot valued at Php350,000.00; and the wrecked vehicle with
depreciated value of Php25,000.00.

 Affidavit of One and the Same Person

Since elementary Bella had been using the name “Maria Isabel C. Lopez”. She is now 18
years old and she wanted to apply for a passport. One of the requirements is a Birth Certificate.
When she secured a copy from PSA, she found out that her name was spelled “Ma. Isabel C.
Lopez”.

 Affidavit of Denial

Engr. Arnold P. Morgan work abroad for 20 years. He returned to the Philippines after the
termination of his contract. Planning to apply for another job abroad, he secured NBI Clearance,
but he was informed that his name was on the “hit list” as he has pending cases before MeTC,
Branch 28, Manila and MTC, Echague, Isabela.

 Affidavit of Good Faith

Spouses Estacio and Clarissa dela Serna obtained a car loan with Chinabank, they wanted to
use the car as security for the payment of their loan obligation.

ABC Corporation held a Zoom meeting on March 1, 2021 where Article 3 of their Articles of
Incorporation was amended by 2/3 votes of its stockholders.

 (Corporate) Secretary’s

ABC Corporation wanted to sue XYZ Corporation, thus in a Board of Directors meeting on
March 20, 2021, the President – Imago Sy was authorized to represent ABC Corporation in
the filing and prosecution of the case that may be filed.
