Consent form

Dear Participants:

You are being invited to participate in a research study about Perception and Extent of engagement
towards persons living with HIV among nursing students in a private university in Iloilo City. This study is
being conducted by Ma. Donna Bornales, Kristine Joy Benicio, Jemima Bodegas and Zyncleir Boskit from
the Allied Health and Sciences Department at University of Iloilo under the advisory of Mrs. Edna P.
Lacre.

There are no risk if you decided to participate in this research study. There are no cost to you for
participating in this study. The information you provide will be kept privately and we assure you that
the data will be gathered from the questionnaire will be used for the purpose of this study alone. The
questionnaire will take about five minutes to complete. The information collected may not benefit you
directly, but the information learned in this study should provide more general benefit.

This survey is google form. No one will be able to identify you or your answers, and no one will know
whether or not your participated in the study. Should the data be published, no individual information
will be disclosed.

Your participation in this study is voluntary. By selecting “ I AGREE” you are voluntarily agreeing to
participate. You are free to pull out your answers anytime.
