MEDICAL REPORT

We, Ike KASHAKA,MD, acknowledge that we conducted medical investigations on Saturday January
3, 2021 around 2 PM on the patient named Rosette TCHANDA aged 30; patient who came for
consultation with the following symptoms:

 Abdominal Pain
 Fever
 Nausea
 Stiffness
 Physical Asthenia
 Headache

After a complete clinical examination and a blood test, we reported evidence of Malaria (objectified
by a positive T. Pallidum and Falciparum rapid test) and typhoid fever (objectified by an ASLO
beyond normal values titrated at 1/320).

To this end, we placed the patient under observation and a treatment consisting of:

- Glucose Serum 500 ml


- Artemether 20 mg / Lumefantrine 120 Mg Iv //3 injections per day for 3 days
- Metronidazol 500 mg in 100 ml IV // 2 infusion per day for seven days.

as been put in place.

In addition to this treatment we recommended a rest of at least two weeks and check-ups after
these two weeks to exclude any risk of relapse, especially relapse which could occur in places where
treatments against malaria, and typhoid are quite rare.
