Vasoactive Agents For Adult Septic Shock: An Update and Review
Abstract
Purpose: The objective of this article is to discuss the pharmacology, side effects, and clinical application of vasoactive therapy in the management of adult septic shock. Summary: Sepsis is one of the most common
reasons for admission to an intensive care unit with the incidence estimated to be greater than 750 000 cases per year in the United States. Clinicians should understand the basic pharmacology of available
vasoactive agents to allow for routine and complex management of septic shock. Conclusion: While advances in research, identification, and early implementation of best practices for the treatment of sepsis
has reduced
mortality, rates remain high. Vasopressors and inotropes remain part of the core therapeutic modalities of sepsis management. Norepinephrine is the first-line vasopressor of choice for septic shock, though secondary
vasopressors can be used depending on the patient’s circumstances.

Epidemiology
Sepsis in the United States includes 2% of all patients admitted to the hospital and 10%
of all ICU admissions.11,12 With

The study by Enrico et al, a study of 23 patients with septic shock, found that dobutamine had an unpredictable effect on systemic hemodynamics. Subjects with septic shock were

included in the study if, per the physician, dobutamine was needed to increase CO if there was any evidence of tissue hypoperfusion.59 Stroke volume index (SVI) only increased in
around half of the patients in the study (P < .02); although HR (P < .00001) and CO (P < .004) increased in most subjects, stroke volume remained unchanged. The greatest increase in
SVI was noted in subjects with echocardiographic evidence of systolic dysfunction.59 In patients with sepsis, dobutamine may cause systemic vasodilation, arterial hypotension, and
tachy- cardia. For these reasons, it is infrequently used, though dobu- tamine plays a role for patients with septic shock with low CO and vascular tone despite adequate fluid
resuscitation.7,47
Methylene Blue
Methylene blue is an alternative agent that can be used as a last resort for the treatment of septic shock when other options have failed.60,61 It may play a role in patients with septic shock
for whom several vasopressors have failed and who show no evi- dence of acute lung injury. 60 The optimal dose for methylene blue is unknown, but most studies evaluated bolus doses
of 1 to 2 mg/kg intravenously as a single dose with a duration of 2 to 3 hours of increased BP.60 Repeated boluses should be based on the patients’ response.60 Methylene blue inhibits
soluble gua- nylate cyclase, thereby inhibiting the vasodilatory effects of nitric oxide.60,61 It has been shown to increase MAP and SVR while reducing vasopressor requirements. 60,61,62
The study by Kirov et al, a randomized, controlled study comparing methy- lene blue to saline, found methylene blue to increase MAP while reducing requirements for norepinephrine,
epinephrine, and dopamine (P < .05).62 There is, however, a concern for increased PVR, methemoglobinemia, and pulmonary vasocon- striction.60 Due to its inhibition of MAO,
methylene blue can cause serotonin toxicity for patients on serotonin reuptake inhi- bitors.63 More studies need to be done.60,61
Angiotensin II
Angiotensin II, a natural hormone with endocrine, autocrine, paracrine, and intracrine effects, is not in the current SSC guidelines but was recently approved by the food and drug
administration (FDA) for septic shock. It has direct vasocon- strictor effects on both arteries and veins, thereby increasing BP. ATII also increases the secretion of antidiuretic hormone
and ACTH and stimulates the release of aldosterone by acting on the adrenal cortex. The half-life of ATII is around 30 sec- onds in the circulation but may be as long as 15 to 30
minutes in the tissues.64
The Angiotensin II for the Treatment of High-Output Shock (ATHOS) trial was a pilot study that randomized 20 patientswith distributive shock to receive an infusion of ATII or
pla- cebo plus standard of care. The primary end point for the study was the effect of ATII on the standing dose of norepinephrine required to maintain a MAP of 65 mm Hg. ATII was
started at a rate of 20 ng/kg/min versus placebo plus the standard of care (norepinephrine starting at 5-10 mg/min) and both were titrated to maintain a MAP of 65 mm Hg. ATII resulted
in a reduction in norepinephrine dosing for all patients (placebo group was
27.6 + 29.3 mg/min vs 7.4 + 12.4 mg/min for the ATII group; P .06). One of the limitations of the study was the small sample size which didn’t allow for adequate power to prove
significance.64 Other limitations are the fact that the inclusion criteria used distributive shock rather than a more specific septic shock and that ATII was compared to placebo rather than
vasopressin which is the current second-line addition.64 ¼
The ATHOS-3 trial randomized 344 patients with vasodila- tory shock who were receiving a usual dose of norepinephrine,
0.2 mg/kg/min or equivalent of another vasopressor, to receive either ATII or placebo. 65 The primary end point was response on MAP at 3 hours after the start of infusion, with response
defined as an increase in MAP of at least 10 mm Hg from baseline or an increase to at least 75 mm Hg without an increase on other vasopressors.65 ATII was effective in increas- ing MAP
in patients with vasodilatory shock that did not respond to high doses of conventional vasopressors (P <
.001).65 There were no significant differences in serious side effects, although further observations are needed to determine possible effects on renal, coronary, and cerebral vascular as there
can be an increased risk of thrombosis.65,66 The study had some limitations such as the inclusion criteria using vaso- dilatory shock rather than a narrower septic shock. 65 The sam- ple
size was relatively small and so it wasn’t powered for mortality reduction or side effects. 65 Future studies will be needed to compare ATII with current treatments such as vaso- pressin to
better learn the role it will play in the care of patients with septic shock.65
Conclusion
Despite advances in research, identification, and early imple- mentation of best practices for the management of sepsis, mor- tality rates remain high. Vasopressors and inotropes remain
part of the core therapeutic modalities of sepsis management. Norepinephrine is the first-line vasopressor of choice for septic shock, though secondary vasopressors can be used depending
on the patient’s clinical picture. Dopamine has fallen out of favor in recent years but continues to play a role for patients with septic shock with a low a risk of arrhythmias and an absolute
or relative bradycardia. Epinephrine can be used as an alternative agent for patients who failed first-line treatments and may be added to norepinephrine for patients who need additional
BP support. Vasopressin may be added to other vasopressors for patients who don’t respond adequately to fluid resuscitation and high-dose vasopressors. Phenylephrine may play a role in
patients with tachyarrhythmias associated with norepinephrine, and dobutamine can be used for patients with
low CO despite adequate fluid resuscitation. ATII has recently been approved by the FDA as an alternative for patients who fail other treatments and is a new addition to the repertoire of
vasoactive agents for septic shock since the last SSC. New studies are needed to tailor its place for the management of sepsis. Methylene blue is used as a last resort when other options have
failed.

Pengantar
Sepsis adalah salah satu alasan paling umum untuk masuk ke unit perawatan intensif (ICU) dengan kejadian diperkirakan lebih dari 750.000
kasus per tahun di Amerika Serikat.1 Angka kematian tetap tinggi disaat dokter berusaha untuk mengatasi hambatan dalam menerapkan terapi ke
dalam praktek klinis.2-4 Vaspressor dan inotropik tetap menjadi bagian dari modalitas terapi inti dari manajemen sepsis seperti yang
direkomendasikan oleh pedoman Surviving Sepsis Campaign (SSC); Namun, literatur terbaru telah menghasilkan perubahan pada rekomendasi

Epidemiologi
Sepsis di Amerika Serikat mencakup 2% dari semua pasien yang dirawat di rumah sakit dan 10% dari semua penerimaan ICU. Dengan kemajuan
dalam pelatihan, pemantauan, dan EGDT, kematian di rumah sakit akibat syok septik telah telah berkurang dari 80% menjadi kurang dari 30%

Norepinefrin, epinefrin, dan dopamin adalah katekolamin endogen, sedangkan katekolamin sintetis termasuk fenilefrin dan dobutamin. Reseptor
adrenergik meliputi reseptor adrenergik a1, b1, dan b2. Reseptor a1 terletak di seluruh pembuluh darah perifer dan terpusat di jantung, dan

Reseptor lain termasuk dopaminergik dan vasopresin
reseptor. Ada 5 reseptor dopamin yang terletak di ginjal dan otak; D1, D2, D3, D4, dan D5. Stimulasi reseptor dopamin ginjal menyebabkan
vasodilatasi pembuluh darah ginjal dan mesenterika. Ada 3 reseptor vasopresin yang ditemukan di seluruh tubuh; V1, V2, dan V3. Stimulasi
reseptor V1, yang terletak di otot polos vaskular, meningkatkan kalsium intraseluler melalui saluran kalsium yang menghasilkan vasokonstriksi
sistemik, splanknikus, ginjal, dan koroner. Aktivasi reseptor V2, terletak di tubulus ginjal, menyebabkan efek antidiuretik. Reseptor V3 terletak
di kelenjar hipofisis anterior dan aktivasi menyebabkan sekresi hormon adrenokortikotropin (ACTH).
Pengamatan klinis menemukan bahwa pasien syok merespon dengan buruk terhadap epinefrin dan norepinefrin ketika terjadi asidosis. Telah
didalilkan bahwa peningkatan pH darah dapat meningkatkan efek epinefrin endogen. Penelitian pada hewan menemukan efek pressor yang
berkurang dari epinefrin dan norepinefrin dengan adanya asidosis. Sebagai hasil dari pengamatan ini, 6 pasien dengan asidosis dan syok
dipelajari dan ditemukan bahwa MAP tetap antara 40 sampai 60 mmHg bahkan ketika menerima norepinefrin dosis tinggi. Setelah pH dinaikkan
dari rendah 7,06 ke tinggi 7,51 setelah pemberian 300 sampai 1000 mL larutan natrium laktat, tekanan darah kembali normal meskipun terapi
vasopressor dikurangi atau dihentikan. Hasil penelitian ini tidak direplikasi oleh hasil Cooper, sebuah studi prospektif, acak, crossover. Empat
belas pasien yang sakit kritis dengan asidosis metabolik dan peningkatan laktat arteri menerima natrium bikarbonat dan natrium klorida secara
berurutan. Tujuh subjek diacak untuk menerima natrium bikarbonat terlebih dahulu. Memperbaiki asidosis tidak berpengaruh pada respon
kardiovaskular terhadap katekolamin atau hemodinamik (interval kepercayaan 95%: 0,06 hingga 0,36 untuk CO; interval kepercayaan 95%: 2,43
hingga 1,85 untuk MAP) . Mengingat temuan ini, pemantauan asam-basa status dan pengobatan asidosis dapat dipertimbangkan pada beberapa
pasien dengan syok septik.
Tujuan dan Pemantauan Terapi Vasoaktif
Tujuan terapi vasoaktif pada syok septik adalah untuk meningkatkan perfusi organ dengan meningkatkan tonus vaskular dan CO setelah
resusitasi cairan telah dioptimalkan. Tujuan terapi hemodinamik adalah spesifik pada pasien, dengan seperangkat pedoman saat ini yang
berfokus pada mempertahankan target MAP. Dalam uji coba baru-baru ini pada pasien dengan syok septik, norepinefrin dititrasi ke MAP target
yang lebih tinggi dari 80 hingga 85 mmHg versus a MAP yang lebih rendah dari 65 hingga 70 mm Hg menemukan tingkat fibrilasi atrium yang
lebih tinggi pada kelompok target yang lebih tinggi tanpa perbedaan dalam mortalitas. Berdasarkan uji coba ini, pedoman SSC saat ini
merekomendasikan target MAP 65 mm Hg atau lebih tinggi. Pasien yang menjalani terapi vasoaktif memerlukan pemantauan terus menerus dari
monitoring statis dan dinamis dari resusitasi cairan untuk menilai respon yang adekuat. Monitoring statis seperti CVP saja tidak lagi secara rutin
direkomendasikan pada semua pasien karena mungkin bukan prediktor yang dapat diandalkan dari respon cairan pada semua pasien. Monitoring
dinamis dari resusitasi cairan saat ini direkomendasikan seperti pengangkatan kaki pasif atau fluid challenge terhadap pengukuran volume
sekuncup jantung. Serum laktat digunakan sebagai pengukuran hipoperfusi jaringan atau efek samping obat yang merugikan dari terapi epinefrin
yang harus dinormalisasi sebagai tujuan terapi. Peningkatan laktat dianggap sebagai bukti hipoksia jaringan dalam keadaan syok, meskipun telah
dibuktikan bahwa hiperlaktatemia yang diinduksi epinefrin dapat disebabkan oleh efek epinefrin pada metabolisme karbohidrat. Pengukuran lain
termasuk BP, HR, saturasi oksigen arteri, RR, suhu, CI, dan keluaran urin (UO).
Agen vasoaktif dapat dikaitkan dengan efek samping kardiovaskular, metabolik, dan vaskular termasuk takiaritmia, bradikardia, dan hipoperfusi
ginjal dan splanknikus. Pilihan agen dan parameter titrasi dapat mempengaruhi perkembangan reaksi ini. Epinefrin telah dikaitkan dengan
gangguan metabolisme seperti hiperglikemia dan asidosis metabolik.
Akses sentral sangat penting untuk pemberian terapi vasopressor, terutama dosis tinggi atau banyak agen. Namun, pada pasien tertentu yang
akses sentralnya tidak memungkinkan, pemberian vasopresor perifer dapat dengan aman diberikan untuk jangka pendek. Banyak agen
vasopresor yang digunakan pada syok septik dapat menyebabkan flebitis dan terjadi ekstravasasi jika diberikan secara perifer. Komplikasi
ekstravasasi vasopressor bisa ringan seperti edema dan eritema, hingga nekrosis jaringan yang membutuhkan pembedahan vaskular atau plastik.
Norepinefrin
Norepinefrin saat ini dianggap sebagai agen lini pertama untuk pasien dengan syok septik yang gagal mencapai MAP 65 mmHg setelah
resusitasi cairan yang adekuat. Karena efeknya pada reseptor adrenergik dengan efek minimal pada B adrenergik reseptor, norepinefrin mungkin
lebih kuat dalam mengatasi hipotensi daripada dopamin sembari menghindari potensi takikardia. Terdapat variabilitas farmakokinetika antara
pasien dalam menanggapi norepinefrin berdasarkan faktor-faktor seperti usia, ginjal , dan fungsi hati. Norepinefrin memiliki onset cepat dan t½
mulai dari 2 hingga 6,8 menit. Norepinefrin dimetabolisme oleh katekol-O-metil transferase dan monoamine oksidase (MAO) menjadi metabolit
inaktif yang diekskresikan dalam urin. A retrospektif, studi kohort membandingkan 100 subjek obesitas, yang didefinisikan sebagai indeks
massa tubuh> 30, dan 100 subjek nonobese setelah infus norepinefrin selama 60 menit. Tingkat rata-rata adalah 0,09 mg / kg / menit pada
obesitas e kelompok versus 0,13 mg / kg / menit pada kelompok nonobese (P 0,006) untuk dosis berdasarkan berat badan dan 8 mg / menit pada
kelompok obesitas versus 9 mg / menit pada kelompok nonobese (P 0,72) untuk non-berat badan Dosis berdasarkan berat badan. Hal ini
menunjukkan bahwa dosis norepinefrin yang diperlukan per berat badan cenderung lebih rendah untuk pasien obesitas dengan syok septik.
Sebuah studi kohort kecil, prospektif, observasi, yang memantau efek norepinefrin pada variabel yang terkait dengan hasil menemukan bahwa
norepinefrin mengalami penurunan yang signifikan dari mortalitas di rumah sakit. Dari 97 pasien dengan syok septik yang dimasukkan dalam
penelitian ini, 57 pasien yang diobati dengan norepinefrin memiliki mortalitas di rumah sakit yang secara signifikan lebih rendah (62% vs 82%,
P <0,001) dibandingkan 40 pasien yang diobati dengan vasopresor lain, meskipun penelitian dibatasi karena desain observasionalnya yang tidak
acak dan berlabel terbuka.
Pemberian norepinefrin awal pada pasien dengan syok septik ditemukan meningkatkan CO menurut Hamzaoui dkk. Seratus lima pasien syok
septik diobati dengan norepinefrin setelah resusitasi cairan dan mengalami peningkatan MAP dan CO yang signifikan sebagai akibat dari
peningkatan preload jantung dan kontraktilitas. Pada pasien dengan uji passive leg raise yang menunjukkan ketergantungan pada preload,
norepinephrine meningkatkan preload jantung dan CI sambil mengurangi ketergantungan preload menurut Monnet dkk. Dua puluh lima pasien
dengan syok septik diberikan uji passive leg raise, kemudian langsung diberikan norepinefrin; setelah hemodinamik stabil, uji passive leg raise
kedua dilakukan diikuti dengan 500 mL saline. Meningkatkan dosis norepinephrine secara signifikan meningkatkan CVP dan CI. Seperti yang
akan terlihat, sebagai hasil dari penelitian ini dan uji coba yang lebih baru , norepinefrin telah menjadi agen pilihan untuk syok septik karena
risiko takiaritmia yang lebih rendah, meskipun vasopresor lain terus berperan dalam keadaan klinis yang berbeda.
Dopamin
Dopamin telah digunakan sebagai vasopressor lini pertama untuk syok septik jika TD gagal mencapai perfusi efektif> 65 mmHg setelah

The Sepsis Occurence in Acutely Ill Patients (SOAP) studi, penelitian observasional kohort, multi-pusat, menemukan tingkat kematian yang
lebih tinggi untuk pasien syok yang menerima dopamin dibandingkan pasien yang tidak menerima dopamin di ICU (42,9 % vs 35,7%; P 0,02)
dan di rumah sakit (49,9% vs 41,7%; P 0,01). Perbedaan dalam angka kematian ini tidak dapat dijelaskan oleh perbedaan tingkat keparahan
penyakit. Penelitian ini melibatkan 3147 pasien, dari dimana 1058 (33,6%) mengalami syok dan 462 (14,7%) mengalami syok septik. Dari
pasien syok, 375 (35,4%) menerima dopamin. Norepinefrin digunakan sebagai agen tunggal pada 31,8% pasien dengan syok.
Studi SOAP I, sebuah uji coba multicenter yang besar dan acak, membandingkan dopamin dengan norepinefrin untuk pengobatan syok tidak
menemukan perbedaan dalam mortalitas, meskipun ada insiden aritmia yang secara signifikan lebih tinggi pada kelompok dopamin. Dari 1679
subjek yang termasuk dalam kelompok Studi, 858 diacak ke kelompok dopamin dan 821 diacak ke kelompok norepinefrin. Tingkat kematian
antara kelompok pada 28 hari adalah serupa, tetapi tingkat aritmia secara signifikan lebih tinggi pada kelompok dopamin (24%; P <. 001) versus
kelompok norepinefrin (12,4%).

syok septik hiperdinamik diacak untuk menerima dopamin mulai dari 10 mg / kg / menit (kelompok I) atau norepinefrin mulai dari 0,5 mg / kg /
menit (kelompok II). Sembilan belas dari 25 subjek di kelompok norepinefrin mencapai sasaran target untuk SBP, indeks SVR, dan CI,
sementara hanya 10 dari 25 pada kelompok dopamin yang mencapai target (P <.05) .
Sebuah meta-analisis oleh De Backer et al dari 5 observasi dan 6 uji coba acak dari total 2.768 subjek membandingkan dopamin versus
norepinefrin untuk pengobatan syok septik. Pemberian dopamin dikaitkan dengan peningkatan angka kematian pada 28 hari (P .035) setelah
salah satu percobaan yang bertanggung jawab untuk heterogenitas telah dieksklusi. Aritmia terjadi lebih sering pada kelompok dopamin
daripada kelompok norepinefrin dalam 2 percobaan yang melaporkan aritmia (P .001) . Setelah dianggap sebagai Agen lini pertama untuk
pengobatan syok septik, dopamin tidak lagi disukai dalam beberapa tahun terakhir karena risiko aritmia bila dibandingkan dengan norepinefrin.
Dopamin terus berperan sebagai vasopresor alternatif pada pasien dengan risiko aritmia yang rendah dan bradikardia absolut atau relatif dengan
syok septik.
Epinefrin
Epinefrin adalah vasopressor alternatif yang dapat digunakan untuk syok septik ketika pilihan lain gagal. Epinefrin adalah agonis adrenergik
poten dan agonis adrenergik b yang meningkatkan CO dan stroke volume dengan beberapa peningkatan tonus vaskular perifer. Namun, ada
kekhawatiran untuk berkurangnya aliran darah splanknikus dan peningkatan kadar laktat. Epinefrin dapat meningkatkan kadar laktat dengan
menurunkan pembersihan laktat hepatosplanchnic dan dapat meningkatkan rasio laktat / piruvat dan kejadian asidosis laktat. De Backer et al
menemukan bahwa epinefrin dapat mengurangi sirkulasi splanknikus dan dapat menyebabkan perubahan metabolik yang besar. Epinefrin dapat
menyebabkan hiperglikemia dengan meningkatkan glukoneogenesis dan glikogenolisis dan juga dengan mengurangi sekresi insulin melalui
stimulasi.
Dalam percobaan CAT II, epinefrin saja dibandingkan dengan norepinefrin plus dobutamin untuk pengelolaan syok septik dalam sebuah
penelitian yang tidak menemukan perbedaan yang signifikan pada kematian atau efek samping. Tingkat takikardia supraventrikular dan aritmia
ventrikel serupa pada 2 kelompok. Penelitian oleh De Backer et al, percobaan prospektif, acak, label terbuka, mengacak 20 subjek dengan syok
sedang atau syok berat. Syok sedang didefinisikan jika dopamin saja dapat mempertahankan MAP> 65 dan syok berat jika dopamin saja tidak
dapat mempertahankan MAP> 65, Dopamin secara bertahap ditarik dari kedua kelompok dan diganti dengan norepinefrin, kemudian epinefrin,
dan urutannya diganti secara acak. Pada pasien dengan syok berat, epinefrin mengakibatkan aliran darah splanknikus lebih rendah secara
signifikan daripada norepinefrin (860 vs 977 mL / menit m2; P <.05) . Profil efek samping epinefrin membatasi penggunaannya sebagai agen
alternatif untuk pasien yang pengobatan lini pertama gagal dan dapat ditambahkan ke norepinefrin untuk pasien yang membutuhkan dukungan
BP tambahan untuk mencapai target MAP.
Vasopressin analog
Vasopresin adalah hormon endogen dan agonis non-selektif V1 langsung yang menyebabkan vasokonstriksi dan meningkatkan kepekaan
terhadap katekolamin. Dalam kondisi fisiologis normal, vasopresin memiliki sedikit efek pada TD tetapi membantu mempertahankan TD selama
hipovolemia. Vasopresin endogen dapat habis pada pasien dengan syok septik dan dapat digunakan pada dosis fisiologis rendah 0,01 sampai
0,04 U / menit untuk pasien dengan hipotensi refraktori. Hipotensi refraktori didefinisikan sebagai SBP <90 mm Hg, MAP <65 mm Hg, atau
penurunan SBP sebesar 40 mm Hg dibandingkan dengan baseline tidak responsif terhadap tantangan cairan. Patel dkk menemukan bahwa
penambahan vasopresin ke norepinefrin mengurangi penggunaan norepinefrin pada syok septik berat sambil meningkatkan fungsi ginjal
dibandingkan dengan norepinefrin saja. Pasien membutuhkan vasopresor dosis tinggi diacak untuk menerima infus norepinefrin atau vasopresin
selama 4 jam dan vasopresor berlabel acak dititrasi untuk mempertahankan tekanan darah. Vasopresor dosis tinggi didefinisikan sebagai dosis
norepinefrin (mg / menit) ditambah dosis epinefrin (mg / menit) ditambah dosis dopamin dibagi 4 (mg / kg / menit) lebih besar dari 5 selama 1
jam. Tingkat norepinefrin menurun dari 20 menjadi 17 mg / menit pada kelompok norepinefrin, sedangkan tingkat norepinefrin menurun dari 25
menjadi 5,3 mg / menit pada kelompok vasopresin pada 4 jam (P <0,001) . MAP dan CI dipertahankan pada kedua kelompok.
Bauer et al menemukan bahwa penghentian vasopresin sebelum norepinefrin mengakibatkan insiden hipotensi yang lebih tinggi pada pasien
yang pulih dari syok septik (P .006) . Lima puluh pasien yang menggunakan norepinefrin dan vasopresin dimasukkan. Tiga puluh dua orang
memiliki norepinefrin yang dihentikan pertama dan 18 telah menghentikan vasopresin terlebih dahulu, namun hanya 5 (16%) pasien dalam
kelompok norepinefrin versus 10 (56%) pasien dalam kelompok vasopresin menjadi hipotensi.
Uji coba thvasopressin dan septic shock (VASST), uji coba multi-center, double-blind, acak dari 778 pasien, membandingkan tingkat kematian
vasopresin ditambah vasopressor label terbuka versus norepinefrin untuk syok septik. Norepinefrin diinfuskan pada kisaran 5 hingga 15 mg /
menit dan vasopresin dosis rendah diinfuskan dengan kecepatan 0,01 hingga 0,03 mg / menit untuk subjek yang menggunakan vasopresor
berlabel terbuka. Keduanya dititrasi untuk mempertahankan MAP pada 65 hingga 75; dokter yang merawat dapat memodifikasi kisaran target
untuk setiap pasien. Tidak ada perbedaan yang signifikan dalam mortalitas pada 28 hari antara kelompok (P 0.26). Analisis post hoc dari uji
coba VASST membandingkan vasopressin versus norepinefrin pada hasil dari cedera ginjal pada syok septik. Berdasarkan kriteria Risiko,
Cedera, Kegagalan, Kehilangan, dan Tahap Akhir (RIFLE) untuk ginjal akut cedera, vasopresin dapat mengurangi perkembangan menjadi gagal
ginjal dan kematian pada pasien dengan risiko cedera ginjal akut dibandingkan dengan norepinefrin.
Uji coba VANISH adalah uji coba multisenter, tersamar ganda, acak dari 409 subjek yang membandingkan efek vasopresin dini versus
norepinefrin pada gagal ginjal pada pasien dengan syok septik. Subjek diacak untuk menerima vasopresin dengan hidrokortison, vasopresin
dengan plasebo, norepinefrin dengan hidrokortison, atau norepinefrin dengan plasebo. Hasil utama adalah hari bebas gagal ginjal 28 hari setelah
pengacakan. Tidak ada perbedaan yang signifikan dalam jumlah penderita yang tidak pernah mengalami gagal ginjal antara kelompok
vasopresin ( 57%) dan kelompok norepinefrin (59%; interval kepercayaan 95%: 13% hingga 8,5%).
Dalam rangkaian kasus 50 pasien dengan syok septik berat, vasopresin tidak menunjukkan efek pada tekanan arteri pulmonalis sistolik,
menunjukkan vasopresin tidak akan memperburuk PVR. Beberapa komplikasi vasopresin termasuk takikardia, penurunan stroke volume, CO,
iskemia, nekrosis pada dosis tinggi, dan aliran darah splanchnic berkurang, yang menghalangi penggunaannya sebagai agen lini pertama. Ini
harus dianggap sebagai terapi pengganti untuk defisiensi relatif dibandingkan dengan vasopressor yang dititrasi untuk efek. Vasopresin dapat
digunakan untuk pasien dengan syok septik setelah resusitasi cairan dan vasopresor dosis tinggi dikombinasikan dengan vasopresor lain untuk
membantu mempertahankan target MAP.
Terlipresin adalah analog sintetik vasopresin yang disetujui untuk digunakan di Eropa, Australia, Selandia Baru, dan India tetapi belum disetujui
untuk digunakan di Amerika Serikat. Terlipresin lebih selektif daripada V1 daripada V2 pada rasio 2.2: 1 dibandingkan vasopresin yang
menderita V1 dan V2 pada rasio 1: 1. Ini mungkin alasan untuk insiden yang lebih rendah dari hipotensi rebound yang terlihat dengan terlipresin
ketika dihentikan dibandingkan dengan vasopresin. Morelli dkk mempelajari 15 subjek dengan syok septik pada norepinefrin dosis tinggi,
didefinisikan sebagai 0,6 mg / kg / menit atau lebih selama minimal 36 jam. Subjek dengan MAP 50 sampai 55 mm Hg diberikan terlipressin 1
mg secara intravena. Terlipresin menghasilkan peningkatan MAP (P <.05), UO (P <.05), dan perfusi mukosa lambung (P <.05) dan penurunan
norepinefrin dosis tinggi (P <.05) yang bertahan selama 24 jam. Terlipresin diberikan melalui intravena karena waktu paruh yang lebih lama dari
4 sampai 6 jam dan telah terbukti secara signifikan meningkatkan MAP pada pasien dengan syok septik walaupun sudah dilakukan penggunaan
norepinefrin dosis tinggi, dopamin, dan resusitasi cairan.
Phenilephrin
Fenilefrin adalah agonis A adrenergik murni yang dapat digunakan sebagai alternatif untuk pasien takikardia karena kemampuannya untuk
menginduksi bradikardia refleks. Fenilefrin memiliki onset yang cepat dan durasi yang singkat serta dapat meningkatkan tekanan darah dengan
sedikit perubahan CO atau volume stroke. Sebuah penelitian kecil menemukan bahwa menambahkan fenilefrin ke dopamin atau dobutamin
dosis rendah meningkatkan MAP dan CI tanpa perubahan pada HR. Ada kekhawatiran akan potensinya untuk mengurangi aliran darah
splanknikus dan mengurangi CO. studi oleh Morelli et al, sebuah studi percontohan prospektif, crossover, menemukan bahwa fenilefrin
menyebabkan vasokonstriksi hepatosplanchnic yang lebih signifikan dan penurunan fungsi ginjal dibandingkan dengan norepinefrin. Lima belas
pasien dengan syok septik yang menggunakan norepinefrin dialihkan ke fenilefrin selama 8 jam, dititrasi untuk mempertahankan MAP antara 65
dan 75 mm Hg, kemudian beralih kembali ke norepinefrin. Penelitian ini menemukan bahwa ada penurunan yang signifikan karena peningkatan
konsentrasi laktat arteri, pada kelompok fenilefrin dibandingkan dengan kelompok norepinefrin pada MAP yang sama (P <0,05). Bersihan
kreatinin juga menurun pada kelompok fenilefrin dibandingkan dengan kelompok norepinefrin (P <0,05). Sebuah penelitian kohort retrospektif
terhadap 27.835 subjek dengan syok septik menemukan tingkat kematian yang lebih tinggi selama waktu kekurangan norepinefrin (39,6% vs
35,9% selama waktu normal; P .03), menunjukkan mortalitas yang lebih tinggi dengan fenilefrin. Fenilefrin adalah vasopresor alternatif yang
paling umum digunakan dalam waktu pendek. Karena terbatasnya jumlah penelitian yang mengevaluasi penggunaan fenilefrin pada syok septik,
obat ini umumnya tidak direkomendasikan tetapi mungkin berperan pada pasien dengan takiaritmia terkait dengan norepinefrin karena
kurangnya agonisme reseptor B.
Dobutamin
Dobutamine adalah inotropik yang berperan pada pasien dengan syok septik dengan CI rendah atau saturasi vena rendah dan MAP yang
memadai setelah resusitasi cairan. Ini adalah satu-satunya inotrop predominan yang direkomendasikan oleh SSC. Dobutamine terutama
bertindak sebagai agonis reseptor b1 dengan beberapa efek campuran pada reseptor a1 menghasilkan respons inotropik. Efek campuran adalah
hasil dari campuran rasematnya dari (-) isomer, bertindak sebagai agonis a1, dan (+) isomer, bertindak sebagai antagonis a1.
Studi oleh Enrico et al, sebuah studi terhadap 23 pasien dengan syok septik, menemukan bahwa dobutamin memiliki efek yang tidak dapat
diprediksi pada hemodinamik sistemik. Subjek dengan syok septik dilibatkan dalam penelitian jika, menurut dokter, dobutamin diperlukan untuk
meningkatkan CO jika ada bukti hipoperfusi jaringan. Indeks volume stroke (SVI) hanya meningkat pada sekitar setengah dari pasien dalam
penelitian (P <.02); meskipun HR (P <.00001) dan CO (P <.004) meningkat pada sebagian besar subjek, stroke volume tetap tidak berubah.
Peningkatan terbesar SVI tercatat pada subjek dengan bukti ekokardiografik dari disfungsi sistolik. Pada pasien dengan sepsis, dobutamin dapat
menyebabkan vasodilatasi sistemik, hipotensi arteri, dan takikardi. Untuk alasan ini, obat ini jarang digunakan, meskipun dobutamin berperan
untuk pasien dengan syok septik dengan CO dan tonus vaskular rendah meskipun telah dilakukan resusitasi cairan yang adekuat.
Metilen Blue
Metilen biru adalah agen alternatif yang dapat digunakan sebagai pilihan terakhir untuk pengobatan syok septik ketika pilihan lain telah gagal.
Metilen biru mungkin berperan pada pasien dengan syok septik dimana beberapa vasopresor telah gagal dan yang tidak menunjukkan bukti
cedera paru akut. Dosis optimal untuk metilen biru tidak diketahui, tetapi kebanyakan penelitian mengevaluasi dosis bolus 1 sampai 2 mg / kg
secara intravena sebagai dosis tunggal dengan durasi 2 sampai 3 jam peningkatan BP. Bolus berulang harus didasarkan pada respon pasien.
Methylene blue menghambat kelarutan nistylate cyclase, dengan demikian menghambat efek vasodilatasi dari nitric oxide. Telah terbukti
meningkatkan MAP dan SVR sekaligus mengurangi kebutuhan vasopressor. Studi oleh Kirov et al, studi acak terkontrol yang membandingkan
metilen biru dengan saline, menemukan metilen biru meningkatkan MAP sekaligus mengurangi kebutuhan norepinefrin, epinefrin, dan dopamin
(P <0,05) . Namun, ada perhatian untuk peningkatan PVR, methemoglobinemia, dan vasokonstriksi paru. Karena penghambatan MAO, metilen
biru dapat menyebabkan toksisitas serotonin untuk pasien yang mengalami inhibitor reuptake serotonin. Penelitian lebih lanjut perlu dilakukan.
Angiotensin II
Angiotensin II, hormon alami dengan efek endokrin, autokrin, parakrin, dan intrakrin, tidak ada dalam pedoman SSC saat ini tetapi baru-baru ini
disetujui oleh administrasi makanan dan obat (FDA) untuk syok septik. Ini memiliki efek vasokonstriktor langsung pada arteri dan vena,
sehingga meningkatkan tekanan darah. ATII juga meningkatkan sekresi hormon antidiuretik dan ACTH serta merangsang pelepasan aldosteron
dengan bekerja di korteks adrenal. Waktu paruh ATII adalah sekitar 30 detik dalam sirkulasi tetapi bisa selama 15 sampai 30 menit di jaringan.
Uji coba The Angiotensin II for the Treatment of High-Output Shock (ATHOS) adalah studi percontohan yang mengacak 20 pasien dengan syok
distributif untuk menerima infus ATII atau plasebo plus perawatan standar. Titik akhir utama penelitian ini adalah efek ATII pada dosis
norepinefrin yang diperlukan untuk mempertahankan MAP 65 mm Hg. ATII dimulai dengan kecepatan 20 ng / kg / menit dibandingkan dengan
plasebo ditambah standar perawatan (norepinefrin mulai dari 5-10 mcg / menit) dan keduanya dititrasi untuk mempertahankan MAP 65 mm Hg.
ATII menghasilkan penurunan dosis norepinefrin untuk semua pasien (kelompok plasebo adalah 27,6 + 29,3 mg / menit vs 7,4 + 12,4 mg /
menit untuk kelompok ATII; P 0,06). Salah satu keterbatasan penelitian ini adalah ukuran sampel yang kecil yang tidak memungkinkan
kekuatan yang memadai untuk membuktikan signifikansi. Batasan lainnya adalah fakta bahwa kriteria inklusi menggunakan syok distributif

Percobaan ATHOS-3 mengacak 344 pasien dengan syok vasodilatasi yang menerima dosis norepinefrin biasa, 0,2 mcg / kg / menit atau setara
dengan vasopressor lain, untuk menerima ATII atau plasebo . Tujuan utama adalah respons pada MAP pada 3 jam setelah dimulainya infus,
dengan respon yang didefinisikan sebagai peningkatan MAP setidaknya 10 mm Hg dari awal atau meningkat menjadi setidaknya 75 mm Hg
tanpa peningkatan vasopressor lain. ATII efektif dalam meningkatkan MAP pada pasien dengan syok vasodilatasi yang tidak respon dosis tinggi
vasopresor konvensional (P <0,001). Tidak ada perbedaan signifikan dalam efek samping yang serius, meskipun pengamatan lebih lanjut
diperlukan untuk menentukan kemungkinan efek pada ginjal, koroner, dan pembuluh darah otak karena ada peningkatan risiko trombosis.
Penelitian ini memiliki beberapa keterbatasan seperti kriteria inklusi menggunakan syok vasilatasi daripada syok septik yang lebih spesifik.
Ukuran sampel relatif kecil dan karenanya tidak didukung untuk pengurangan mortalitas atau efek samping. Penelitian di masa depan akan
diperlukan untuk membandingkan ATII dengan pengobatan saat ini seperti vasopresin untuk mempelajari lebih baik perannya dalam perawatan
pasien dengan syok septik.
Kesimpulan
Meskipun ada kemajuan dalam penelitian, identifikasi, dan penerapan awal praktik terbaik untuk pengelolaan sepsis, angka kematian tetap
tinggi. Vasopresor dan inotropik tetap menjadi bagian dari modalitas terapi inti manajemen sepsis. Norepinefrin adalah vasopresor lini pertama
pilihan untuk syok septik, meskipun vasopresor sekunder dapat digunakan tergantung pada gambaran klinis pasien. Dopamin tidak lagi disukai
dalam beberapa tahun terakhir tetapi terus berperan untuk pasien dengan syok septik dengan risiko aritmia yang rendah dan bradikardia absolut
atau relatif. Epinefrin dapat digunakan sebagai agen alternatif untuk pasien yang gagal dalam pengobatan lini pertama dan dapat ditambahkan ke
norepinefrin untuk pasien yang membutuhkan dukungan BP tambahan. Vasopresin dapat ditambahkan ke vasopresor lain untuk pasien yang
tidak merespons resusitasi cairan dan vasopresor dosis tinggi secara memadai. Fenilefrin dapat berperan pada pasien dengan takiaritmia yang
terkait dengan norepinefrin, dan dobutamin dapat digunakan untuk pasien dengan CO rendah meskipun telah dilakukan resusitasi cairan yang
memadai. ATII baru-baru ini telah disetujui oleh FDA sebagai alternatif untuk pasien yang gagal dalam pengobatan lain dan merupakan
tambahan baru pada agen vasoaktif untuk syok septik sejak SSC terakhir. Studi baru diperlukan untuk menyesuaikan tempatnya untuk
pengelolaan sepsis. Methylene blue digunakan sebagai pilihan terakhir ketika opsi lain gagal.
