Health Perception and Health Management Pattern

The Parents of the patient stated that general health prior to


admission has been described as good unless symptoms arise. General
health prior and during admission has been distressing because of its
recurrent symptoms. The patient is brought to the RHU for treatment
medications for the relief of symptoms when present. Cold, cough and
fever were common prior to admission and symptoms have been affecting
daily work. The patient’s daily activity is helping his parents in
their farm and is thought to be his exercise. Patient has no complete
immunizations. Patient has no history of accidents. Patient follows
suggestions and orders from the doctors and nurses in the RHU. Upon
admission the police suspect’s that the patient might be taking
illegal drugs. During admission, the patient is manic and is on four-
point restraint.

Nutritional metabolic pattern

The Parents of the patient mentioned that typical food intake prior to
admission is foods present in their farm which are fruits and
vegetables. For their protein, it is usually chicken and water for
hydration. Soft drinks were uncommon because it is far from their
home, usually native coffee from their farm is present. There were no
supplements taken. The parents mentioned that there was no weight loss
or gain with the patient. Prior to admission, the patient has a good
appetite despite his symptoms arising. The patient has allergies on
peanuts and seafoods. During admission the patient is not showing good
appetite because of the spike and severe symptoms present. No food
discomfort or swallowing problems prior and during admission. No skin
problems and dental problems evident.

Elimination Pattern

The Parents of the patient mentioned normal bowel elimination pattern


prior and during admission. Parents mentioned that they usually see
the patient go to the restroom every day without any reports of
discomfort, problems or unusual reactions. Urinary eliminations are
normal prior and during admission. No report of discomfort and
problems coming from the patient. The patient urinates several times a
day without reports of problems. Upon initial interview with the
patient, the patient stated normal bowel pattern, character and
frequency. The patient also mentioned normal urination pattern,
character and frequency. No evident discomforts and problems were
noted coming from the patient.

Activity and Exercise Pattern

The parents of the patient stated that prior to admission the patient
has sufficient energy required for his daily activities. Daily
activity pattern is helping in the farm and is regularly the patient’s
pattern which is also a form of exercise. Patient’s spare time is
usually taking a rest or sleep. All of these events are affected when
symptoms are present. When symptoms are present, functional levels are
affected including, feeding, grooming, bathing, toileting, bed
mobility, health maintenance, dressing and mobility. Parents mentioned
that the client needs assistance from them when his symptoms are
present. During admission, symptoms were affecting the patient’s
normal actions and energy. During admission, the patient is highly
manic and aggressive in which he is restraint. Functional levels are
affected during admission because of the spike of symptoms.

Sleep and Rest Pattern

The parents of the patient mentioned that prior to admission the


patient appears to be generally rested and is ready to help them in
their farm. Sleeping patterns were normal and the patient sleeps 7-8
hours daily, patient is seen to be calm when symptoms were not
present. When symptoms are present at home, the patient’s sleeping
pattern is affected and is showing signs of aggressive actions during
night time, this is relieved with medications. During the admission
and the peak of symptoms, the patient has not been sleeping, appears
to be not rested and clear symptoms of having lack of sleep. Patient
just takes a short nap and continues to be awake until his body gives
up.

Cognitive and Perceptual Pattern

The parents of the patient mentioned normal cognitive and perceptual


pattern when symptoms are not present prior to admission. During
admission the patient is showing clear signs of hallucinations and
delusions. Patient is perceiving a conversation with another person
which is not present. Patient is perceiving to be pointing and
touching someone while yelling and shouting who is not present.
Patient could not talk properly to anyone and does not answer
questions appropriately. Patient could not remember events in life.
Patient could not take decisions and is struggling in every aspect of
thinking and cognition. Patient is having psychotic episodes prior and
during admission. During admission, the patient is very angry and
agitated and becomes very loud and begins threatening the Emergency
department staff.

Self-Perception and Self Concept

The parents of the patient described him being confident and happy
prior to admission and when symptoms where not present, but the
patient knew that he has this disorder that is affecting his
cognition. Patient does not have bodily changes that are problematic
to him. During admission, Patient is distressed towards himself
because of the experiences he is having with the condition. Patient
mentioned clear delusions that made him angry, fearful, and depressed
about. Patient mentioned that he has lose hope and is experiencing
things in his mind that could not be explained.
Roles and Relationship

The parents of the patient mentioned that the patient has a good
relationship with them. The patient is the only son and is living with
them. They also mentioned minor family problems including financial
problems which they are trying to solve daily. Their only main concern
is when the patient manifest psychotic symptoms affecting their daily
living since the patient is the only child to help them in the farm.
The parents are already used to this situation since the patient is
manifesting it for the past years and their solution is to bring their
son to the RHU to ask for medications. The parents mentioned that this
is the daily cycle of their son, helping in their farm in the
mountains. The patient has friends but does not interact a lot with
them because he is busy. Parents of the patient mentioned that they
are sometimes scared when the symptoms are severe because since the
patient is aggressive and can hurt someone.

Sexuality and Reproduction

The parents of the patient mentioned that they did not see their son
with a partner. Upon admission the patient mentioned that he has not
had a partner since then. In addition, the patient stated that he Has
not had any relationships nor is planning to have any sexual
relationships with the opposite sex. He is currently single because of
the disorder he is carrying that he cannot control.

Coping and Stress Tolerance

The parents of the client mentioned that a big event that occurred in
their family is when the patient was diagnosed with schizophrenia.
This happened when the patient was on his college years and it gave a
big impact to the family because of the change of lifestyle and the
fact that he was their only child that could have finished college.
Upon admission, the patient mentioned that he is thankful for his
parents for guiding him every day and that he wanted to be in a normal
state. The patient mentioned that problems are present every day,
including financial problems but the important thing is that they can
eat three times a day. The patient mentioned that prior to admission,
when his symptoms are present, his parents bring him to the RHU for
injectable medications for his symptoms.

Values and Belief 

The patient mentioned that the goals in life are not easy to achieve,
he wanted to be successful in order to help his parents escape from
poverty, but the patient felt hopeless because of the condition he is
carrying. The patient mentioned that religion is a part of life. The
patient does not seem to give a big importance towards religion. The
patient mentioned that he sometimes goes to church and only prays when
he has problems or needed things. The patient mentioned that prayers
helped him in some ways.
