SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, JUAN DELA CRUZ Filipino, single, of legal age, residing at #1 City do by these
presents, hereby NAME, CONSTITUTE and APPOINT MARIA MAKILING likewise
Filipino, married, of legal age and resident of #1, Baguio City to be my true and lawful
ATTORNEY-IN-FACT, for me and in my name, to do and undertake the following acts:

1. To APPEAR, SIGN and PROCESS documents on my behalf in all transactions

pertaining to my resignation as an employee of the Baguio General Hospital and
Medical Center particularly at the Human Resource Department of the BGHMC;
2. To EXECUTE, SIGN and PROCESS pertinent documents to different government
offices such as the GOVERNMENT SERVICE INSURANCE SYSTEM (GSIS) in
relation to the above transaction;
3. To SIGN and RECEIVE Checks or Cash due to me from the BAGUIO GENERAL
HOSPITAL AND MEDICAL CENTER, GSIS and BGHMC Multi-Purpose Credit
Cooperative;
4. And to do or perform any act or acts necessary for the accomplishment of the above-
mentioned powers.

HEREBY GIVING AND GRANTING UNTO the said Attorney-In-Fact full power and
authority whatsoever requisite or necessary or proper to be done in or about the premises as fully
to all intents and purposes as we might or could lawfully do if personally present, with full power
of substitution and revocation, and hereby ratifying and confirming all that our said attorney-in-
fact shall lawfully do or cause to be done under and by virtue of these presents.
