AFFIDAVIT OF EXPLANATION

I, CHARITO RAGASA CALVO, of legal age, single, Filipino and a resident of

Nagsangalan, Vigan City, Ilocos Sur after having been duly sworn to in accordance with
law, do hereby depose and says:

That I was a holder of a Philippine Passport issued by the Department of Foreign

affairs in Manila sometime in the year 1999;

That I used and brought said passport when I travelled to the United States
sometime in 2012;

That said passport during my stay at 736 – Rimgrove La Puente, California

91744 was lost and I cannot remember anymore how and when I lost said passport
because I got stroke twice last January 2016 and February 2016;

That I exerted all efforts to locate the same but can no longer be found and I now
consider it as beyond recovery and lost forever;

That said passport was not confiscated or held by authorities;

That I am executing this affidavit to attest to the truth of the foregoing facts and in
order to secure a new passport from DFA and for all legal intents and purposes.

AFFIDAVIT OF EXPLANATION

I, EDMEL PASCUA PESCADOR, of legal age, married, Filipino and a resident of

Malingeb, Bantay, Ilocos Sur after having been duly sworn to in accordance with law, do
hereby depose and say that:

1. I am a holder of Philippine Passport with passport No. EC1216260 issued by the

Department of Foreign Affairs La Union valid until May 25, 2019;

2. I am in the process of facilitating the renewal of my passport in San Fernando

City, La Union;

3. I am requesting the Department of Foreign Affairs La Union for the early renewal
of my passport due to the fact that I am to be deployed overseas and will not be
able to renew my passport overseas due to the nature of my work therein;

4. Thus, I am opting to renew my passport early to ready such passport for

immediate travel abroad and comply with company requirements;

5. I am executing this affidavit to attest to the truth of the foregoing statement and
for all legal purposes it may serve;

6. I hereby assume full criminal, civil and administrative liability in case of any
action, charges, suit, proceeding, prosecution or investigation that may be filed
against me or any person by any court of justice, tribunal, board or agency,
quasi-judicial body or office should any of the document or paper I submitted be
found dubious, spurious or fake, or should any of my statements and
declarations be found to be false or unfounded;

7. I shall waive any and all rights, actions, charges, suits, claims and cases against
the Department of Foreign Affairs or the government or any of its officer or
employee in case of denial or approval of this application, or in case of damage,
injury or death arising from any travel that I may undertake during, after, pursuant
to or consequent to this application, nor in case of any action, suit, proceeding,
prosecution or investigation that I may be subjected to by virtue of this
application;

8. This affidavit is executed to attest to the truthfulness and veracity of my

statement above.
