AFFIDAVIT OF SUPPORT

That I, ELIZA S. URBAYO, of legal age, single Filipino citizen, with residence and
postal address at Brgy. Refaro, San Juan, Ilocos Sur after having been duly sworn to in
accordance with law, hereby depose and state:

That I am the daughter and the sister respectively of the visa applicants: OFELIA
PURISIMA SALVIO and JAMES CHRISTIAN S. URBAYO born on September 6, 1958 and
September 26, 1996 in San Juan, Ilocos Sur traveling under Philippine Passport No.
P5341961A valid until December 13, 2022 and Philippine Passport No. P1097650A valid until
December 4, 2021;

That my mother and brother will be travelling with me to South Korea from
____________________;

That I have sufficient income/assets to responsibly cover my mother and brother’s

expenses during our entire trip to South Korea, including return flights to the Philippines, as
well as any and all other unforeseen expenses which my mother and brother may incur
during our stay in South Korea;

As a daughter and sister, I hereby accept and assume any and all responsibility for
their welfare, including all financial expenses related to their travel and stay in South Korea;

I guarantee that they will abide by all laws and regulations of the South Korean
government;

That I am executing this affidavit to attest to the truth of the foregoing facts for all legal
intents and purposes.
