AFFIDAVIT OF SERVICE
       I, JEFFREY PASCUA, of legal age, married, Filipino, and presently residing at
Barangay Camangaan, Vigan City, Ilocos Sur, after having been duly sworn to in
accordance with law, depose and state:

1. That I am presently employed at the Local Government Unit of Vigan City- Office of
the City Public Safety and Disaster Risk Reduction and Management with the
position of Administrative Aide III;

2. That on or about January 4, 2018, our Department Head, Mrs. Milagros Flora
Alquiza issued a Office Memorandum No. 3, Series of 2018 address to Mr. Miguel
Adolfo, one of the Traffic Enforcers of the City; Attached hereto is the Memorandum
No. 3, Series of 2018 marked as Annex “A” and formed an integral part of this
Affidavit;

3. That being the Office Clerk, I was assigned to serve the above-mentioned
Memorandum to Mr. Adolfo;

4. That in compliance to the said order, I served the same at our Office on the same
date at 3:40PM;

5. Upon service of the said Memorandum to Mr. Adolfo, the latter after reading and
accepting the same, audaciously refused to sign the receiving copy thereof; he even
texted me reiterating his refusal to receive; Attached hereto is the picture of the text
message of Mr. Adolfo marked as Annex “B” and formed an integral part of this
Affidavit;

6. Because of such refusal, I noted such development on our receiving copy and sign
the same after affixing the date and time of such service;

7. That I declare and certify that Mr. Adolfo refuses to receive the Memorandum No. 3,
Series of 2018, as such, I execute this affidavit to attest to the truth of the foregoing
facts and statement and for whatever legal purpose it may serve.

       I, MELCHOR REBOLDERA, of legal age, married, Filipino, and presently residing
at Barangay Bongtolan, Vigan City, Ilocos Sur, after having been duly sworn to in
accordance with law, depose and state:

1. That I am presently employed at the Local Government Unit of Vigan City- Office of
the City Public Safety and Disaster Risk Reduction and Management with the
position of Traffic Aide;

2. That my area of responsibility is Divine Word College of Ilocos Sur (Burgos Street) to
Tongson Royal Bibingka (Florentino Street);

3. That on or about January 4, 2018 at around past 6:00 in the evening, I saw and had
a conversation with Mr. Miguel Adolfo at Tongson Royal Bibingka establishment;

4. That Mr. Adolfo during our conversation mentioned that our Department Head, Ms.
Milagros Alquiza issued a Memorandum against him and that he told me that he
refuses to receive the same;

5. That I declare and certify that Mr. Adolfo during our conversation admitted that he
refuses to receive the Memorandum issued by Ms. Milagros Alquiza, as such, I
execute this affidavit to attest to the truth of the foregoing facts and statement and
for whatever legal purpose it may serve.
