AFFIDAVIT OF ONE AND THE SAME PERSON

duly sworn to in accordance with law hereby depose and state:

1. That I am using the name xxx since birth;

2. That while my Original Certificate of Live Birth bears xxx as my name, it was
later changed to xxx; (See Attached Certificate of Birth, Pursuant to the
decision rendered by xxx)
3. That all my Government Records and Identification Cards subsequent to the
approved changed of name bear the name xxx;
4. That my family and friends know me asxxx;
5. That when I first applied for Philippine Passport on June 22, 2015, I used the
name xxx, as my Certificate of Live Birth at that time has not yet been changed
to xxx;
6. That when I applied for another Philippine Passport on October 14, 2016, I used
my true and corrected name, which is xxx;
7. That I do not intend to use the name xxx in all my transactions and
representations;
8. That I intend to use xxxD in all my transactions and representations as my true
and correct name;
9. That while xxx AND Lxxxare one and the same person, I intend to cancel my
Philippine Passport bearing the name xxx and intend to renew my Philippine
Passport bearing the name xxx;
10.That I am executing this affidavit to attest to the truth of the foregoing and to
cause the cancellation of my Philippine Passport bearing the name xxx and to
renew my Philippine Passport bearing the name xxx.
