AFFIDAVIT OF ACKNOWLEDGEMENT OF PATERNITY

I, BENJAMIN NAGAC BAGUIO, JR., Filipino, of legal age, and

a resident of Kauswagan, Cagayan de Oro City, Misamis Oriental,
after having been duly sworn to in accordance with law, depose and
state:

1. That I am the biological father of BENJ DOMINIQUE

TAMPOS BAGUIO, who was born outside of wedlock on December
29, 2019 at Tandag City, Surigao del Sur;

2. That at the time of birth of said child, I was not married to

her mother, Jay Mariz Tampos Tampos;

3. That I acknowledge my paternity/affiliation to the said

child;

4. That I am executing this Affidavit to attest the foregoing

facts and for whatever legal purposes this may serve.

IN WITNESS WHEREOF, I have hereunto affixed my
