AFFIDAVIT OF LOSS

I, NIKO JAY S. PAJARITO, of legal age, single, Filipino citizen, and with residence at
Zone IV, Arcillias Street, Mercedes, Zamboanga City, Philippines, after having duly sworn in
accordance with law, do hereby depose and state that:

1. That I am the holder of receipt no._______________, issued by the ASUS Concept

store, located at 2nd floor KCC Mall de Zamboanga;

2. That sometime on March 2021, I discovered that my receipt no._______________,

issued by the ASUS Concept store, was probably lost or misplaced;

3. Despite Diligent search and efforts to locate the said RECEIPT, I could not find the
same that I now believe that it is now lost beyond recovery;

4. As such that I am executing this affidavit of loss to attest to the truth of the foregoing
and to support for my warranty privilege.
