AFFIDAVIT

I, ROLANDO A. DEL MAR, of legal age, Filipino and resident of Sanson Rd.
Lahug, Cebu City, Philippines, after having been sworn in accordance with the
law hereby depose and state:

1. That I am born 05 December 1948 and have used the same in my

personal and official records and interactions, as shown by my
PhilHealth ID and Senior Citizen ID, copies of which are hereto
attached as Annex “A” and “B”;

2. That my relatives, acting on the honest belief that I was born on 05

December 1948, had used 05 December 1948 as my birth date in all
pertinent documents relative to governmental services and other
personal transactions;

3. That my date of birth was erroneously recorded by the Vicente Sotto

Memorial Medical Center (VCMMC) in Cebu City, Cebu, as “12 May
1948”;

4. That I am executing this affidavit to attest to the truthfulness of the

foregoing statements and to establish that my date of birth is 05
December 1948 and for all legal intents purposes it may serve.
