8:51 PM The patient had 5 bouts of loose bowel movement,
Body Tempt 38 degree Centigrade . PR 65 RR 20,
Stool is water based, with foul smell, flushed face,
warm to touch, Had history of eating ice cream ,cake
in a bday party.
ACTION:
Administer Loperamide (Imodium) to slows the
movement of food through your intestines, which
lets your body absorb more liquid. The patient is 5 yr
old the dosage is 7.5 mL (1½ teaspoons) after first
loose stool (2-3 times a day)
RESPONSE:
After 4 hours of administering the medication the
patient, was able to report reduction in frequency of
stools.

Scenario:

A 5 yr old girl was brought by the mother to ER with the cc of LBM. Upon interview patient had 5 bouts
of loose bowel movement, Body Tempt 38 degree Centigrade. . PR 65 RR 20, Stool is water based, with
foul smell, flushed face, warm to touch, Had history of eating ice cream , cake in a bday party.

Make a charting .

Intolerance
8:51 PM The patient had 5 bouts of loose bowel movement, Body Tempt 38
degree Centigrade . PR 65 RR 20, Stool is water based, with foul
smell, flushed face, warm to touch, Had history of eating ice
cream ,cake in a bday party.

The symptoms shows signs of intolerance to milk and other dairy


products. Diarrhea is a typical indication of lactose intolerance.
Patients with lactose intolerance have insufficient lactase, the
enzyme that digests lactose.

ACTION:

Since Lactose Intolerance don`t have a medication, advise the


parent to limit the intake of dairy products to the child`s food
intake.

Administer lactase enzyme tablets or drops. This helps digest dairy


products. The patient can take tablets just before a meal or snack.
Or the drops can be added to a carton of milk

RESPONSE:

After 4 hours of administering the medication the patient, was able


to report reduction in frequency of stools.

degree Centigrade . PR 65 RR 20, Stool is water based, with foul
smell, flushed face, warm to touch, Had history of eating ice
cream ,cake in a bday party.

The symptoms shows signs of food poisoning, because of the fever


and diarrhea. Food borne illness are usually infectious or toxic in
nature and caused by bacteria ,viruses, parasites or chemical
substances entering the body through contaminated food or water
arise from improper handling, preparation or storage of food.

ACTION:

Administer Oral rehydration salts: If diarrhea persists, oral


rehydration solution as ORS, should be drunk to replace the fluid
losses and prevent dehydration.

RESPONSE:

After 4 hours of administering the medication the patient, was able


to report reduction in frequency of stools.
