MEDICATIONS  Instructed to take the following take home
medications at the right dose, right time,
right frequency and right route;
1. Insulin 70/30 Subcutaneous injection
every 12 hours:
 15 units subcutaneous injection
30 minutes pre-breakfast (6:30
am)
 18 units subcutaneous injection
30 minutes pre-dinner (6:30
pm)
2. Metformin 500 mg tablet + Vildag liptin,
1 tablet twice a day (8:00 AM and 6:00
PM)
3. Atorvastatin 40 mg tablet, 1 tablet once
a day before bedtime at 8:00 PM
4. Carvedilol 6.25 mg tablet, 1 tablet twice
a day (8:00 AM and 6:00 PM)
5. Clopidogrel 75mg + ASA 75 mg tablet, 1

tablet once a day in 8:00 AM in the


morning
6. Trimetazidine 35 mg tablet, 1 tablet
twice a day (8:00 AM and 6:00 PM)
7. Captopril 25 mg tablet, ½ tablet twice a
day with BP precaution (8:00 AM and
6:00 PM)
8. Pantoprazole 40 mg tablet, 1 tablet
once a day 30 minutes before
breakfast
9. Amlodipine 10 mg 1 tablet, once a day
6:00 PM in the afternoon
10. Losartan 50 mg tablet, 1 tablet once a
day in 8:00 AM in the morning
 Encourage to comply with medications to
prevent further complications
EXERCISE  Aim for at least 30 minutes of exercise
each day.
 Encourage for a deep breathing exercise
 Passive ROM as tolerated;
o Neck flexion/extension
o Wrist flexion/extension
o Leg flexion/extension
o Ankle flexion/extension/rotation
TREATMENT  Make sure to have a regular check-up as
instructed by your physician
 Seek medical advice if any unusual arises
 Keep your blood sugar at a target level
and as often as your doctor recommends
 Record and manage the details about your
chest pain and see for medical advice
HEALTH TEACHINGS  Daily wound dressing 2 x a day with
Bactroban ointment
 CBG monitoring pre breakfast and pre
dinner
 Eat a good diet; avoid foods with high
sodium, high in sugar, oily foods, starches
rather eat vegetables, and fruits
 Take your medicines as exactly as
prescribed
 Bed linen must be dry and clean
 Improvise side rails in the bed of patient
 VS bp monitor daily
 Turn your patient side to side 2 hours
 Eatong position high fowlers agpalpa 2
hours
 Keep on track of any symptoms such as
having a low blood sugar, changes in your
activities, diet or insulin use
 Keep your cholesterol and blood pressure
at normal level
 Check your feet each day for sores
 Do not trim your toenails
 Avoid activities that causes angina
 Medication compliance
OBSERVABLE SIGN AND SYMPTOMS  Advise immediately to seek medical
assistance when experience symptoms of
unstable angina;
o Shortness of breath
o Chest pain
o Nausea/Vomiting
o Discomfort pain in your
back/neck/stomach/arm
o Lightheadedness or cold sweat
 Advise immediately to seek medical
assistance when experience symptoms of
low/high blood sugar;
 Sweating
 Weak
 Extreme hunger and nausea
 Blurred vision
 Dizziness and headache
 Confusion
DIET  Do not skip meals
 Low fat; avoid food such as dairy products
 Low salt diet; avoid food such as instant
noodles, canned goods, dried fish
 Use the plate method to help you with
portion control; ½ of vegetable, ¼ of
starches like potatoes, and ¼ of meat
SPIRITUAL CARE  Continue to seek God’s guidance and
enlightenment
 Pray to God for the past recovery
 Continue to have a positive outlook in life
 Keep faith in God and not to give up easily
 Make sure to go to church every Sunday
