How many benefit plans are there in your organization?
(Eg: Medical, Vision etc)
Are all the above benefit plans same across all the legal entities?
List down all the qualifying life events that impact the enrollment into various plans?
(Eg: Marriage, New Child, Divorce, Death etc)
List down all the Options available to an employee for various plans (Eg: Medical Plan
Options : Employee Only, Employee plus spouse etc)

How many plans are unrestricted? (i.e an employee can change the elections any time)
How many plans are based on qualifying life event (i.e an employee can change
elections only after a qualifying life event?)
When does the ‘Open Enrollment’ takes place in the organization?
How many carriers or vendors are there for these benefit plans? Please list each of
them here with their corresponding plan names.
How do you generally send across the files to the Benefit Vendors? (FTP/SFTP, email
etc)
What is the frequency of providing the files to the Benefit vendors?
Please provide specification files and sample extract files for each of the benefit
vendors.
Do you also send COBRA information along with the benefit plan enrollment
information to vendor?
For each of the benefit files, please specify whether you send the full file or ‘changes
only’ file.
Cut off dates for submission of these benefit files to the vendor?
