AGREEMENT

I, ______________ (Name of student), resident of ________________________, has

VOLUNTARILY enrolled in a certain Review Center. I hereby express my agreement to
the following Terms and Conditions set by the Center relative to the availed review
courses/program:
1. The student agrees to fully, actively and religiously cooperate in the review
activities and processes designed by the reviewers.
2. The student voluntarily subject himself/herself to the Review Process designed
by the reviewers from the review content, method, assessment and board exam
qualification requirements.
3. The student agrees to continuously and diligently continue with the Review
Process until he/she is able to qualify for Board Examination.
4. The student agrees that his/her enrolment to the Review Program does not
guarantee that he/she will be allowed to take the board examination.
5. The student fully entrusts the reviewers in setting the final requirements that
qualify the student to take the Board Examination. Such requirement includes the
board qualifying passing grade to be met by the Reviewees in the series of pre-
board examination to be administered in the review process.
6. Nobody shall be liable for any undesirable incident that the student/s may
commit/encounter while enlisted in the review process.
