Q1 The disappointing OSH record in the major organization works such as construction,
mechanical and chemical works has been highlighted. It is important to systematically
pursue the Occupational Safety and Health Management System Implementation
(OSHMS) in this organization.

(a) Briefly explain FIVE (5) main elements of OSHMS implementation in the
organization.
(3 marks)

(b) Consider SEVEN (7) health and safety performance measurements to evaluate
the safety and health culture performance in the organization.

Q2 (a) Hazards in the workplace differs every day. In order to effectively manage
workplace safety and health, proper systems and procedures need to be
introduced to ensure hazard identification is carried out regularly. State
THREE (3) situations when hazard identification process needs to be performed
and repeated.

(b) If a worker painting a room, a step stool must be used to reach higher areas
where he is not allowed to stand higher than 1 metre or 3 feet at all time. The
assessment team reviewed the situation and agrees that working from a step
stool at 1 metre is likely to cause certain problems where the problems refer to
Severity and Probability respectively. Based on this situation,

Q3 (a) An electrical shock occurs when a person encounters an electrical energy source
that flows through a part of the body. Electrical energy exposure can result in
no injury at all or can also cause severe harm or death. Describe the different
paths of electric flow in the human body that influences the nature of injuries.
(4 marks)

(b) A worker was trying to see what is wrong with the electrical circuit that led to
the non-functional lamp. Unfortunately, he was electrocuted when he grabbed
the live wires with his left hand. He collapsed on the floor and was found dead.
Based on the situation, analyse in detail THREE (3) safe working procedures
that should have been put in place to prevent the accident.
