KNOW ALL MEN BY THESE PRESENTS:

WE, ALEXANDER L. DAYPUYAT and SYLVETTE C. DAYPUYAT, bothof legal ages, Filipino
citizens, married to each other and with residence address at P-2, Kiburiao, Quezon,Bukidnon, after
having been sworn to in accordance with law, hereby depose and state:

1. That we are the owners of property covered by TAX DECLARATION NO. 11-014-317
which was used as collateral to a previous obligation with the closed RURAL BANK OF
QUEZON (BUKIDNON), INC. (RBQI);

2. That we are authorizing Mr. RAY ARISTEO C. OSUNA to represent us, sign and receive
whatever documents that may be necessary for the cancellation of mortgage annotated
on said TAX DECLARATION NO. 11-014-317;

3. That we hereby RELEASE AND DISCHARGE said closed RURAL BANK OF QUEZON
(BUKIDNON), INC. (RBQI) or its Liquidator, its officers, employees, agents or
representatives from any furtherliability, whatsoever, emanating directly or indirectly
from the Release of the Mortgage on our property, and for this purpose we hereby hold
the said closed bank, its Liquidator, its officers, employees, agents or representatives,
legally free and harmless from any further liabilityarising from the foregoing
transaction, claim or incident.
