WEEKLY HSE REPORT

PT. BENINGKHATULISTIWA
PROJECT NAME : METROPOLIS PROJECT - KRAFT HEINZ ABC - KARAWANG
DATE : 4/30/2021
WEEKLY PERIOD : #01 (26 April - 30 April 2021)

A. SUMMARY HSE WEEKLY REPORT

1. HIGHLIGHTS :

1.1 On 30 November 2020 MR. David wattiemena was conduct internal meeting for socialization and preparation of External audit ISO
45001 : 2018 & ISO 9001 2015. In the meeting, all divisions were assigned their respective duties and responsibilities to ensure all documents
were complete before the audit

1.2 On 30 November 2020, FABS HSE team with BVI consultant was inspection heavy equipment excavator PC 78 for demolition works at
Pasteurizer area. The concern of inspection : Clear all maintenance tools from pedal area & electronic devices area (obstacles), Request for
provide good enough fire extinguisher from Civil contractor. The inspection based on Excavator checklist.

1.3 On 1 December 2020 MR. David Wattimena & all Mechanical Engineer was conduct Working At Height (WAH) training especially for
Electrical & piping team before sprinkler & electrical installation (critical working at height +- 12 meter). The concern of this training is about
how the initial procedures and what completeness must be considered to do WAH and step by step how to do WAH properly and correctly.

1.4 On 4 December 2020 all FABS HSE team was conduct manual handling training to all workers. This training discussion is about how to do
manual handling with the correct steps to avoid injuring the muscles / body, and information maximum load capacity for 1 person to do manual
handling.

3. HSE ISSUES :

UNSAFE CONDITION CORRECTIVE ACTION

THERE IS NO FIRE EXTINGUISHER IN FABRICATION AREA ELECTRICAL COORDINATION TO ELECTRICAL SUBCONTRACTOR HSE TO ALWAYS
TEAM. (FIND BY FABS LOCATION ON SOY FABRICATION AREA) PROVIDE GOOD FIRE EXTINGUISHER IN FABRICATION AREA.

ADMINISTRATIVE CONTROL CORRECTIVE ACTION


THERE IS FOUND HEAVY EQUIPMENT EXCAVATOR THAT NO HAVE DISPLAY CHECKLIST, SIA & SIO AS AN IDENTIFICATION FOR HEAVY
PAPER CHECKLIST AS AN IDENTIFICATION AND SIA & SIO AS A LEGAL EQUIPMENT EXCAVATOR
FROM GOVERNMENT. (FIND BY FABS, LOACTION AT PASTEURIZER
AREA).

UNSAFE CONDITION CORRECTIVE ACTION

THERE IS FOUND UNCLEAN EYE WASH AT SOY AREA (FIND BY FABS) COORDINATION WITH CIVIL SUBCONTRACTORS FOR CLEANING EYE
WASH AND ALWAYS UPDATE THE CHECKLIST.

4. INITIATIF / CHALLENGE

4.1 Monitoring all working at height activities especially at SOY and DC area for demolition existing canopy, brick wall installation, and
mechanical electrical work.

4.2 Collect all tools register data from all subcontractors include the checklist inspection.

4.3 Completeness and check & review HIRADC for preparation audit on 17 & 18 December 2020.

4.4 Checked man power monitoring board at DC, Chili, & SOY, and make sure was update daily basic.

4.5 Continuously Safety Induction every Monday, Wednesday, & Friday to all new workers.

4.6 Maintain all HSE subcontractors to always do a team work while start work activities at the same place.

5. MILESTONE & ACCOMPLISHMENT


1. Daily Toolbox Meeting was conducted very consistent and very effective. It was conducted in two separated session. First is General
Toolbox Meeting together with all FABS's team and second is Toolbox only between specific team with specific topic. After tool box meeting
all workers who working at height immediately do a blood pressure test. Tool box meting in this week fabs HSE team was conduct muscles
stretching activities.

2. FABS HSE team was information & coordination to all HSE subcontractors, consultant and client about "Safety Working Pass" helmet
sticker to all workers & staff as a mandatory used to identify the name of workers & staff and how many workers / staff was have done
training.

3. On 3 December, MR. David Wattimena & All HSE team was checked all HSE document for preparation audit ISO 45001 : 2018 on 17 & 18
December 2020.
4. FABS HSE team has been socialization about scaffolding procedures include step by step how to taged a scaffolding while weekly safety
patrol activities.

5.Demolition exisiting canopy area has been protected and provide sign for all workers & staff aware and not entering demolition area.

6. Working at height (WAH) training was conduct on 1 December 2020 & Manual handling training was conduct on 4 December 2020 by FABS
HSE team.

7. Heavy equipment in this week has been inspected & approved by fabs HSE & BV consultant. (excavator PC 78)

Tool Box Meeting Topic Are :


1. Stretching fungction information & always information about covid 19 protocol procedures
2. Weather information to all workers for preparation clear all electrical tools whine rainy.
3. Make sure and information all workers to take shelter from the rain at permanent building and not at trees or Temporary roof.
4. How important communication between subcontractors / work division.
5. All man power has the right to reprimand / stop work if finds some unsafe act / condition.

6. Information to all workers about fungction of Scaffolder. nothing can change the scaffolding except the scaffolder.
7. Green tag scaffolding must be sign from competent person.
