11. 5 Passport size photographs.
12.Police verification by SP --- Original and copy
13. Photocopy of last 2 pages of logbook

14. Payment through Bharat koshof Rs. 5000/-, No ********** Dated ****** ,Payable New-Delhi, in Favor Of
PAO DGCA MCA Payable New-Delhi.

It is requested that the license may be issued at the earliest and the originals be returned .
