I, Mrs. Asuncion Cabo, a resident of Brgy. Malaubang, Ozamiz City would like to request for a clear copy
of the Tax Declaration of my property located in the same barangay. This is because my son, Mr.
Angelito Luar Cabo, also a resident of Brgy. Malaubang would apply for business permit as a woodstick
retailer.

In connection to this, I authorize Ms. Cherry May Dumaog to process the application and release of the
copy of my Tax Declaration including payment of necessary fees.
