    (MIDDLE NAME)

    We, TERESITA L. DOMINGO , of legal age, widow, Filipino citizen and a


    resident of Brgy. Casabaan, Cabangan, Zambales and AUGUSTO ESPAÑOL , also
    of legal age, married and likewise residing at Brgy. Casabaan, Cabangan,
    Zambales, after having been duly sworn to in accordance with law, jointly
    depose and say; THAT:

    We personally know NORBERTO D. FORCADILLA , because he is our


    neighbor and common friend;

    We are aware that he is the son of MARIO FORCADILLA and CLARITA


    DACUMOS , born on JULY 26, 1954 and his birth is recorded under Registry
    Number 3200 of the Local Civil Registrar of Gubat, Sorsogon;

    We are also sure that he is a member of the Social Security System (SSS);

    In his SSS, his date of birth was erroneously recorded as JUNE 6, 1954
    and the registered name of his mother is REMEDIOS DACUMOS ;

    The erroneous entries were cause by inadvertence and oversight when


    he filled-up the form;

    The name NORBERTO D. FORCADILLA whose date of birth indicated in


    Birth Certificate Registry No. 3200 as JULY 26, 1954 with his mother named
    CLARITA DACUMOS and NORBERTO FORCADILLA whose date of birth
    appearing in SSS as JUNE 22, 1954 with his mother named as REMEDIOS
    DACUMOS , pertain to one and the same person, who is our common friend, the
    correct entries of which are those stated in his Certificate of Live Birth;

    We finally state that we are not in any manner related with him within
    the fourth civil degree of consanguinity or affinity nor do we have any
    pecuniary interest in executing this affidavit;

    In witness whereof, we hereunto set our hands this 13 t h day of May 2021
    at Cabangan, Zambales.
