AFFIDAVIT OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

            I, Sino Po Sila, Filipino citizen, of legal age, married and a resident of Brgy. Mailum,
Bago City Neg Occ after having been duly sworn in accordance with law, hereby depose and
say:

1. That on May 31, 2021 I filed for resignation and my intention to be re-appointed as a
regular guard.
2. That respective candidates for the position, SO Ayaw Ko Po and SO Ayaw Ko Na
both declined the position relinquished by SO Sino Po Sila.
3. That anent to the above, another Security Officer will be fielded in lieu of SO Rios.
4. That due to this additional manpower, the existing number of duties of 12-13 days per
personnel per 15 days, will be reduced to 11-12 days per personnel.
5. That on June 2, 2021 RSPSOS for Western Visayas, Bakit Ayaw Mo us of the
situation.
6. That I fully understand the exigency of the situation to fully comply with the client’s
requirements and balance it with agency’s admin and operational requirements.
