Evaluation Form (Directive)

Purpose

The purpose of this document is to set out guidelines for appraising the performance of the GUC
Interns

General Information

 The evaluation form shall provide a supportable basis for making decisions such as
continuation of the internship, Training needs and/or future employments.

 The Supervisor should provide advice support and guidance to enable the intern to achieve
objectives.

Procedure

 The Intern will complete Part I and Part III and forward it to the supervisor.
 The supervisor/mentor is responsible for filling in Part II.
 An evaluation discussion shall take place between the Supervisor and the Intern
 After the discussion both the Supervisor and the Intern will date and sign.

Rating Scale

 Excellent
 Above Average
 Satisfactory
 Below Average
 Unsatisfactory
