1 Introduction

Intended Use of syngo.plaza

With the version VA20B, the intended use of syngo.plaza has been extended:
syngo.plaza is a Picture Archiving and Communication System intended to
display, process, read, report, communicate, distribute, store, and archive
digital medical images, including mammographic images.
It supports the physician in diagnosis and treatment planning.
For primary image diagnosis in Mammography only uncompressed or non-
lossy compressed images and only preprocessed DICOM "For Presentation"
images must be used. Also monitors (displays) and printers which received
FDA clearance for Mammography must be used.
syngo.plaza also supports DICOM Structured Reports.
In a comprehensive imaging suite, syngo.plaza integrates Hospital/Radiology
Information Systems (HIS/RIS) to enable customer-specific workflows.
syngo.plaza optionally uses a variety of advanced postprocessing applications.

Web-based image distribution is not intended for reporting.

10 syngo.plaza
Introduction 1

The Program Modules

The program interface is designed on the card index principle (syngo
standard). Its structure reflects the workflow at an image viewing and
archiving workplace.

The number of functions on your syngo.plaza workplace depends on the

licenses available, and on your personal user privileges.

Task cards
The main applications are set up as task cards, and are easily accessed using
the tabs at the edge of the screen.

Viewer ■ You can view images on the Viewer task card, and if necessary, edit them.
The Viewer offers a wide range of functions for displaying, viewing, and
evaluating images.

CD-R and Archive ■ Depending on your license, the CD-R, or Archive task card is shown. This
task card enables you to create patient media, start manual archiving, and
to manage the archive database.

Scanner ■ On the Scanner task card, you can digitize images, such as X-ray films or
reports. The digitized images can then be forwarded to other physicians,
for example, over the intranet or the Internet.

Filming ■ Finally, you can also output digital images on film with the Filming task
card.

Embedded 3D ■ On the 3D task card, 2D images are merged to form a volume. New views
and slices within a volume can then be generated and evaluations made.

Addendum 11
1 Introduction

Further modules

Patient List ■ The Patient List for selecting and managing the patient data can be
accessed from all of the task cards. The Patient List comes to the
foreground on application start-up.

MWL Scheduler ■ The MWL Scheduler enables you to schedule appointments for
examinations on each configured modality.

Report Viewer ■ You can display reports from the →RIS with the Report Viewer.
With the DICOM SR Viewer, you can display DICOM Structured Reports.

syngo MammoReport ■ You can load Mammography images directly from syngo.plaza to a syngo
(Option) MammoReport workplace. syngo MammoReport is an advanced system
for reading and reporting high-volume mammograms. The application
offers an optimized workflow, which is suitable for both screening and
diagnostics.

The rules of safety apply to clinical users as well as to administrators.

This syngo.plaza VA20B Safety Advisory chapter supplements the Safety
Advisory chapters in the Operator Manual and in the Administrator Manual of
syngo.plaza VA10A.
To operate syngo.plaza safely, you should carefully read the Safety Advisory
chapters in this Addendum, in the syngo.plaza VA10A Operator Manual and in
the syngo.plaza VA10A Administrator Manual.
It is assumed that syngo.plaza is operated by qualified personnel only.

Addendum 15
2 Safety Advisory Addendum

Additional Rules of Safety for Clinical

Users
To guarantee the proper functionality of syngo.plaza, the following important
information has to be observed carefully.

Synchronization of Study Data

Study data cannot be synchronized, when the data is locked, for example
during auto archiving, or reading in the Viewer. Changes made within this
CAUTION time frame are not synchronized and therefore cannot be immediately
displayed .
If a synchronization fails, it can be done manually by the administrator. After
a manual synchronization, changed and synchronized study components
will be in status nearline. These data must be dearchived once again.
Risk of incorrect diagnosis due to an incomplete study
◆ Please check the error logs and have your system synchronized manually
by your administrator, if necessary.
◆ Open your study from the Patient Jacket, where data in nearline and
online storage is indicated.

Cardio Thoracic Ratio (CTR)

The precision of measurement results may be limited by several factors, for

example, the use of anterior-posterior images, a measurement inaccuracy,
CAUTION or rounding errors.
Risk of incorrect diagnosis due to incorrect measurement results
◆ Consider this effect when interpreting measurement results.
◆ Take into account that the center line is not based on anatomic
structures but on the image dimensions.
◆ Check the plausibility of measurement results.

16 syngo.plaza
Safety Advisory Addendum 2

Manual Calibration of Images

After manual calibration, the calibration value specified in the Calibration

dialog box may differ from the value displayed on the calibration line.
CAUTION Risk of incorrect distance values due to image pixels that are not square
◆ Consider this effect when manually calibrating images.
◆ Consider that all subsequent measurements are based on the displayed
value.

When adding new images to a series of calibrated images, the calibration is

not automatically applied to the new images. The resulting consequence
CAUTION may be a combination of calibrated and non-calibrated images.
Risk of incorrect diagnosis due to incorrect measurement results
◆ Consider this effect when interpreting measurement results.
◆ Check whether calibration is applied to all images.
◆ If necessary, recalibrate the series after having added new images.

Image Quality of Reference Images

Reference images included in the Structured Report cannot be displayed in

diagnostic quality.
CAUTION Risk of incorrect diagnosis due to insufficient image quality
◆ For diagnostic purposes, always send the images as DICOM objects.
◆ Always display the images with an adequate hardware and software.

Addendum 17
2 Safety Advisory Addendum

Additional Rules of Safety for

Administrators
To guarantee the proper functionality of syngo.plaza, the following important
information has to be observed carefully.

system crash.
CAUTION Loss of messages to or from the RIS/HIS
◆ Only set up a RIS/HIS that can buffer and return messages.
◆ Archive all messages from or to the RIS/HIS for subsequent manual
resend, if required.
◆ If the existence of images is not indicated although the respective study
is available, always check HL7 messages for the completeness of images.

18 syngo.plaza

Layout Parameters for Mammography

For reading mammograms, syngo.plaza provides a workflow support: you can
use or modify default layouts, or create your own layouts. Each layout
corresponds to a step within the diagnostic procedure. After creation, you can
save a set of layouts as a user-specific workflow.
Layouts are based on the following parameters:
■ Number of monitors available (1 to 4)
■ Layout of image segments (1 x 1, 1 x 2, 2 x 2, etc.)
■ View positions (for example, LM, ML, MLO, CC)
■ Laterality (L, R)
■ Prior-Study configuration
This configuration enables you to perform a comparison between the
current study and prior studies.
For more information on the workflow support, see “Creating and Modifying
Mammo Layouts” in the Online User Documentation.

Calculating the Cardiothoracic Ratio

(CTR)
You can measure the cardiothoracic ratio (CTR) to determine, for example, the
cardiac enlargement in pediatric patients.
CTR is the ratio of heart's maximum horizontal diameter to the maximum
width of the thoracic cavity.
9 A suitable image is selected
Prerequisite for CTR calculations is a standard PA (posterior-anterior) view
of Computed Radiography (CR) images and of Digital Radiography (DX)
images.

In the CTR mode, you can measure the CTR by drawing four lines on the image,
in parallel to a center line, in order to delimit the heart and the thorax.

Starting CTR Calculation 1 Click the Cardio Thoracic Ratio icon on the tool palette:

The CTR mode is activated and a center line is displayed in the middle of
the image.

3 To determine the transverse diameter of the thorax, click the thorax’s right
and left edge.
Two vertical lines delimiting the thorax are displayed in the image, in
parallel to the center line.
Once the four lines have been drawn, the CTR value is calculated and displayed
as a text annotation centered at the bottom of the image.
The result is added to the Findings Navigator.

Adjusting the Center 4 To adjust the center line to the orientation of the image, click the center
Line line and rotate it until the required position is reached.

The four measurement lines are rotated as well. During rotation, the angle
of the centre line always determines the angle of the measurement lines.

Adjusting Measurement 5 To adjust a drawn measurement line to the anatomic structure, click the
Lines line and drag it to the required position.
The CTR value is recalculated and displayed centered at the bottom of the
image.

When saving the changes, all markings are saved as a Presentation State

DICOM LTA 6

A DICOM Long-Term Archive (LTA) enables the long-term storage (LTS) of

DICOM Composite Objects (DCOs), such as images or Structured Reports.
syngo.plaza supports Vendor Neutral Archiving (→VNA) scenarios that help to
reduce the administrative effort of managing the LTS of different products.
syngo.plaza provides you with two different archiving solutions:
■ DICOM Archive Provider (→DAP)
You can use syngo.plaza as DICOM Archive for other PACS systems.
■ DICOM Archive User (→DAU)
You can use syngo.plaza as DICOM Archive for the long-term storage (LTS)
of DICOM objects.

DICOM Archive Provider During a radiological workflow, all data has to be available on the short-term
storage (STS) for fast access. When the work is done, data will be transferred
from the STS to the LTS. This LTS can be a NAS/DAS system, or a DICOM Archive
Provider (DAP).
The syngo.plaza DAP provides DICOM Archive functionality for other PACS
systems to store, query, and retrieve data, for example, images, or Structured
Reports. Thus, syngo.plaza enables a single storage solution for multiple
vendors. Data can be de-archived without any loss or change of information.

DICOM Archive User In the case of a large hospital, it is possible that an isolated DICOM Archive
Provider is positioned decoupled from potentially multiple PACS front-end
solutions. syngo.plaza DICOM Archive User (DAU) functionality can be
deployed to provide radiological reporting functionality while archiving to the
neutral DICOM Storage or DICOM Archive Provider.

Magic Compatibility Module

The images remain archived in the long-term storage (→LTS) of SIENET
MagicStore. The content of the SIENET Magic database is imported into
syngo.plaza. When accessed from syngo.plaza, the images are converted into
the syngo.plaza image format.

When migrating legacy data, the Magic Compatibility Module (MCM) may
incorrectly map display information objects to image sets.
CAUTION Risk of incorrect diagnosis due to incomplete or faulty mapping of
legacy information
◆ Always check migrated data for consistency most carefully.
◆ In case of doubt, do not use the data for diagnosis.

After data transition, SIENET Magic data sets are accessible and displayed
within syngo.plaza. So you can apply the functions of syngo.plaza to SIENET
Magic data sets.

When migrating legacy data, the Magic Compatibility Module (MCM) may
incorrectly map legacy ACRNEMA/DICOM database fields in SIENET Magic to
CAUTION the DICOM/syngo.plaza database scheme.
Risk of incorrect diagnosis due to incorrect or missing mapping of
legacy database fields
◆ Always check migrated data for consistency most carefully.
◆ Do not use images for diagnosis that indicate insufficient information
due to data loss.

For further safety advice on data migration, see “Data Migration from SIENET
Magic” on page 19.

MCM Data Migration 7

Data Transition
For the transition of data from the SIENET Magic database to the syngo.plaza
database with MCM, several options are available.

MCM Transition by Fast Switch-Over

9 The MCM Transition is processed by fast switch-over.
The MCM Transition by fast switch-over includes the following general
procedure steps:

General Procedure Steps ■ Database information is uploaded to syngo.plaza.

■ The STS (→STS) of syngo.plaza is filled from the LTS (→LTS) of SIENET
Magic.
■ The RIS is interfaced with syngo.plaza.
■ syngo.plaza information is uploaded to the RIS.
■ SIENET Magic is shut down.

During Data Transition ■ Patient data is imported from the SIENET Magic PDIR database to the
syngo.plaza database.
■ After the database upload, the STS of syngo.plaza does not contain
images.
■ Images from the LTS of SIENET Magic are nearline and have to be
prefetched either manually or RIS-triggered.
Via MCM, the images, that had been archived in the LTS by SIENET
MagicStore, can be accessed .
■ In a final step, the RIS is updated, including the database extraction and
the import into the RIS.

Addendum 47
7 MCM Data Migration

MCM Transition in Parallel Operation

9 The MCM Transition is processed in the background, SIENET Magic and
syngo.plaza are operated simultaneously.
The MCM Transition in parallel operation includes the following general
procedure steps:

General Procedure Steps ■ The RIS is connected with both PACS systems in parallel.
■ Database information is uploaded to syngo.plaza and the RIS.
■ The STS (→STS) of syngo.plaza is filled with relevant images from the LTS
(→LTS) of SIENET Magic.
■ Some modalities point to syngo.plaza, or autoroute is activated.
Working with syngo.plaza can be started while SIENET Magic is still in use.
■ All modalities point to syngo.plaza. SIENET Magic is shut down.

During Data Transition ■ The transition is performed department-wise. Once, all studies from all
modalities of a department are transferred, the system is configured for
MCM use. All studies of this department are read in syngo.plaza while
studies of other departments are still read in SIENET Magic.
■ All studies on the LTS of SIENET Magic can be accessed via MCM, once they
have been uploaded to syngo.plaza.
■ Continuous so-called delta uploads to the database are necessary to
upload the patients and studies that have been modified, or received after
the initial database upload into syngo.plaza.

For more information on how to perform delta uploads, contact your

Siemens Migration Engineer.

Please note that exceptions always have to be fixed in both systems.

48 syngo.plaza
MCM Data Migration 7

RIS in Parallel Operation

9 The RIS communicates with both PACS systems in parallel.
The simultaneous communication of the RIS with both PACS systems includes
the following general procedure steps:

General Procedure Steps ■ The RIS receives notifications from SIENET Magic and syngo.plaza.
■ The RIS launches studies in SIENET Magic and syngo.plaza.

In the case that the RIS is unable to operate with two PACS systems
simultaneously, only one PACS system can use the RIS work list for image
call-up.

Important Information ■ Patient and study updates should always be performed on the RIS.
on the RIS
■ If a third party RIS is used, the RIS may be unable to support notifications
that are sent by syngo.plaza. Additionally, the RIS may not use internal
syngo identifiers. In this case, the notifications from syngo.plaza to the RIS
may be turned off.
■ If a RIS integration is missing, all patient modifications have to be
performed manually in SIENET Magic.
By configuration of the database upload tool, the patient information from
SIENET Magic overrules the patient information of syngo.plaza.

Data Transition Status

SIENET Magic data sets are indicated by the text label MCM.
If there was a problem with the MCM conversion of a particular study, this is
indicated by an ERROR status in the following places:
■ Journal of the Patient List
■ Journal of the Archive/CDR task card
■ Access Bar of the Viewer

If an entire study cannot be converted, it remains nearline. No ERROR status

is displayed in this case.

version is available at http://dicom.nema.org.
DSA. Digital Subtraction Angiography is a type of fluoroscopy technique used
in interventional radiology to clearly visualize blood vessels in a bony or dense
soft tissue environment. Images are produced using contrast medium by
subtracting a 'pre-contrast image' or the mask from later images, once the
contrast medium has been introduced into a structure.

L LTA. DICOM Long-Term Archiving enables the long-term storage (LTS) of

DICOM Composite Objects (DCOs), such as images or Structured Reports.

M MTS. Mid-Term Storage. Custom file format that enables the joining together
of data files without compressing them. Thus, a specific DICOM file can be
read instantly, instead of extracting the MTS file to a temporary location
before accessing the data.

P PIX. Patient Identifier Cross Referencing. The Patient Identifier Cross

Referencing (PIX) Integration Profile supports the cross-referencing of patient
identifiers from multiple Patient Identifier Domains.

R RIS. Radiology Information System. Administration system for the radiology

department, which takes care of patient administration, documentation,
acknowledgment of services, and the writing of reports.

S Same Size. In this mode, all loaded images of the same patient are displayed
in the same relative physical size, for example, two objects of the same real
physical size appear in the same size in the Viewer.
SR. Structured Reports. Structured Reports (SR) are documents with a defined
structure used for reporting results or evidence. They provide the basis for
exchanging reporting data, e.g. DICOM data, from various applications in a
common, standardized format.
STS. Short-Term Storage. STS bases on storage technologies and connections,
which allow for a quick access to the image data. Usually a RAID system
connected with the archiving server is used as an STS.

T TCP/IP. Transmission Control Protocol/Internet Protocol: Transmission

protocols for data exchange between different computers.
True Size. In this mode, all loaded Mammography images are displayed in
their real physical size in the Viewer.

V VNA. Vendor Neutral Archiving helps to reduce the administrative effort of

managing the LTS of different products.

W Waste Basket. By assigning the option Add Waste Basket to a layout, it is

ensured that all mammography images can be viewed. This option enables the
image display even if the image parameters do not correspond to the layout of
any workflow step within the designated workflow.
