Client’s name: Jade Williams

Presenting problem: Jade indicated she is looking to build greater self-efficacy, become more

adept with technology, get away from gang life to change her lifestyle, and foster a sense of

independence.

Goal

1. Build greater self efficacy by identifying strengths and learning how to keep a schedule

over the course of 5 weeks.

Objectives

1. Client will write a resume within the next two weeks, highlighting her skills and

previous work experience, in order to build self-efficacy.

2. Client will identify 5 strengths and track those strengths in a journal as she uses them

in daily routine.

Interventions

1. Client will bring the resume into session. Counselor will provide her with

feedback and constructive criticism. Will also discuss her identified strength and

experiences to build greater confidence.

2. The client will participate in a “Card Sorts” exercise and take the “VIA Strengths

Survey” to help her identify some of her strengths and interests.

Goal

2. Client will identify ways to get away from gang life to establish healthy relationships and

friendships.

Objectives

1. Client will make a list of pros and cons of what it was like being in the gang culture

as that was her “only family”

2. With information revealed in the “Card Sorts”, client will begin to seek out 2-3

groups that have similar values and interests.

Interventions

1. Client will identify two or three AA or NA groups she is interested in attending.

2. Psychoeducation on trauma and the effects of it on the mind and body will be

presented to the client to help her have a better understanding about how it has

ruled decision making in the past. Identify and practice healthy coping strategies

in session to improve calming strategies.

These resources may help the client access affordable food and healthcare. When she is on her

own she may need help feeding herself and accessing healthcare on a budget. This is meant to

help her cover those basic needs.

client options open and find the best fit for her.

group might better suit her needs and help her build a sober community that is more supportive

of her sobriety and changing life than her gang family.

Client was provided a website list of NA meetings to include the side of substance abuse. This

can help with feelings of being othered as she will be able to be with like-minded individuals

seeking to better their lives.
