A. Formulate sample items for the following types of test:

1. Multiple Choice with Stimulus Materials (20 Items Multiple Choice)


2. Cluster True or False (10 items True or False)
3. Sequencing Matching (3 sets composing 5 elements each)
4. Filling the Blanks Completion (10 items)
5. Essay

B. Ask for or borrow sample test questions from a teacher and analyze how the questions
were formulated. Your comments will be kept confidential between you and your
instructor to avoid future disputes if ever you provide unfavorable comments to the
aforesaid material.

In making a test paper and reading some article in making a test questionnaire. I’ve read that there
are two general categories of test items. First, the objective items which require students to select
the correct response from several alternatives or to supply a word or short phrase to answer a
question or a complete statement; the second is subjective or essay items include the multiple
choice. True or false, matching and completion, while subjective items include short answer, essay,
extended-response essay, problem solving and performance test items.
The sample test paper above, the teacher uses the two categories. The first part of the test
paper is multiple choice a pre-test to evaluate the preparedness of the students for further studies.
The teacher say that this assessment is non-graded test based. The second part is essay they need
to connect their lifestyle in the topic that has been discuss to them. It teaches them to communicate
through writing.
