AFFIDAVIT OF LOSS

I, HAZEL STELLE POTOT ALONZO, of legal age, single, Filipino and a

resident of Mercedes, Zamboanga City, after being duly sworn to in accordance with law,
depose and say THAT;

1. I am in possession and holder of that Senior Citizen Identification Cards

(OSCA) of my grandparents, namely to wit;

a. ROSA MAGTANGOB ALONZO, who was born on August 30, 1949

at Basilan, Philippines;

b. NICOLAS L. ALONZO, who was born on August 16, 1941 at Ipil,

Zamboanga Sibugay, Philippines;

2. That as Senior Citizen, and such they was issued by the Office of the Senior Citizen
Affairs Identification Card with bearing No. ________________&_________________,
issued on _______&________ at Zamboanga City, Philippines;

3. That sometime on January 25, 2021, I discovered that the aforementioned Senior
Identification cards was probably lost or misplaced;

4. Despite diligent search and efforts to locate the said Senior ID’s, I could not find
the same such that I now believe that it is now lost and beyond recovery;

5. That I am executing this affidavit to attest to the truth of the foregoing narration of
facts, more particularly to declare the loss of the Senior ID’s, and for whatever
legal purpose this may serve.

I, HADJA MARIAM ABBILANI, of legal age, married, Filipino and a resident

of Sta. Barbara, Zamboanga City, after being duly sworn to in accordance with law,
depose and say THAT;

1. I am in possession and holder of an Official Receipt with EPEB No.

496/2021005492 dated 02-08-2021 upon paying for the Transfer Fees with the
amount of Fourteen Thousand Seven Hundred Ninety Nine and Fifty Six
Centavos (Php.14,799.56) issued by the Register of Deeds for Zamboanga City;

2. That I misplaced the said Official Receipt with EPEB No. 496/2021005492 dated
02-08-2021, got lost and cannot be located and after exerting diligent effort to look
for the same, it cannot be located and is deemed lost beyond recovery;

3. Despite diligent search and efforts to locate the said Official Receipt with EPEB
No. 496/2021005492 dated 02-08-2021, I could not find the same such that I now
believe that it is now lost and beyond recovery;

4. That I am executing this affidavit to attest to the truth of the foregoing narration of
facts, more particularly to declare the loss of Official Receipt No. with EPEB No.
496/2021005492 dated 02-08-2021, and for whatever legal purpose this may
serve.
