JOINT AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, LIGAYA S. PABILONIA and HENRY AMONELO PABILONIA, Filipino,
both of legal age and a resident of Municipality of Cainta, Province of Rizal, under oath,
do hereby depose and state, that:

We personally know MA. ISABEL CONCEPCION VICEDO (“Ma. Isabel”);

We know for a fact that Ma. Isabel was born of the Spouses LYDIA VIBAT
BUCAO and LIBERATO VALENCIANO CONCEPCION on JANUARY 20, 1966 in
Condelaria, Quezon and was named MA. ISABEL BUCAO CONCEPCION. A copy of
her Certificate of Live Birth is hereto attached as Annex “A” and made an integral part
hereof;

On April 11, 2011, Ma. Isabel contracted marriage with JOSEPH A. VICEDO
under civil rites at the Office of the Municipal Mayor of Biñan, City. A copy of their
Certificate of Marriage is hereto attached as Annex “B” and made an integral part
hereof;

On September 06, 2018, Ma. Isabel succumbed to Cardiopulmonary Arrest at

Manila East Medical Center. A copy of her Certificate of Death is hereto attached as
Annex “C” and made an integral part hereof;

It so happened, however, the following discrepancies were found:

a. As per her Certificate of Death and Certificate of Marriage, her given name
was registered as MARIA ISABEL instead as MA. ISABEL CONCEPCION
VICEDO;
b. As per, her mother’s name was inadvertently registered on her Marriage
Contract and Death Certificate as MARIA LYDIA VIBAT BUCAO instead of
LYDIA VIBAT BUCAO;
c. As per, her registered on her Application Provident Benefits Claim her
father’s name was LEVY V. CONCEPCION instead of LIBERATO
VALENCIANO CONCEPCION;
d. As per, her father’s name was inadvertently registered on her Certificate of
Live Birth and Certificate of Marriage as LIBERATO SO CONCEPCION
instead of LIBERATO VALENCIANO CONCEPTION;

We are not related to Ma. Isabel either by affinity or consanguinity;

We know the foregoing facts because we are close friends and neighbors of their
family since then up to the present;

This affidavit is being executed to attest to the veracity of the foregoing

statements in order to establish MARIA ISABEL CONCEPCION VICEDO as
 per Certificate of Death and Certificate of Marriage and MA. ISABEL
CONCEPCION VICEDO Certificate of Live Birth and SSS Records refers to
one and the same person. And that the correct and true father’s name is
LIBERATO VALENCIANO CONCEPCION and true and correct mother’s
name is LYDIA VIBAT BUCAO and for purposes not contrary to law, morals,
good custom and public policy;
