NARRATIVE

Preliminary investigation revealed, On June 23, 2021 at approximately 1155 hours, Vehicle-1 was traveling southbound on I-295 in the #3 (far right)
lane. The Operator of Vehicle-1 reports he had just left a nearby concrete plant adjacent to I-295 and forgot to lower the vehicle’s subframe. As Vehicle
-1 drove under the footbridge that traverses I-295, the subframe struck the footbridge which caused the footbridge to partially collapse on top of the
vehicle’s subframe as it passed. Vehicle-1 came to final rest partially on the north side and partially on the southside of the footbridge.

Vehicle-2 was traveling southbound in the #2 (middle) lane next to Vehicle-1. As Vehicle-1 collided with the footbridge, Vehicle-2 swerved, hit the left
jersey barrier and was able to travel completely south of the footbridge at which point a large piece of concrete from the footbridge fell on top of
Vehicle-2’s roof and front windshield, where it came to final rest.

Vehicle-3 was traveling southbound in the #2 (middle) lane, slightly behind Vehicle-1. As Vehicle-1 collided with the footbridge and it began to collapse
, the highway exit sign for “Benning Rd and Foote St” fell on top of the hood of Vehicle-3, where it came to final rest completely north of the
footbridge.

Vehicle-4 was traveling southbound in the #1 (far left) lane when the footbridge began to collapse after Vehicle-1 struck it. Vehicle-4 struck Vehicle-3
and came to final rest completely north of the footbridge.

Four of the five involved persons were transported to area hospitals for non-life threatening injuries.
