LGU Assessment Tool for COVID-19 Response

Technical Notes on How to Use the Data Collection App

Description: The data collection app was developed to automate the recording and submission of the
LGU assessment reports on COVID-19 Response. This document provides the step-by-step guide
on how to install the data collection app, how to encode an assessment report, and how to edit a
submitted report form.

I. Installing the LGU Assessment Tool Data Collection App

If you have previously installed the a.) via Device Setting

 Long press the Data Collection App icon
(BlueStacks)  Look for uninstall option

c.) via Laptop/Desktop


 Go to Settings >> Application
 Right click the Data Collection App icon and choose
uninstall
(Data Collect App/ CMELRCR)
 Right click the BlueStacks/Android emulator icon and
choose uninstall

Downloading & Installing an Android Emulator (for Apple or Windows device)


If you are using an Apple or
Windows device, install an Android
emulator (e.g., BlueStacks) before
downloading the app.

To download BlueStacks,

1. Go to
https://www.bluestacks.com
and click on “Download
BlueStacks”

2. Click “Install”

If the emulator is successfully installed,


you will see the Bluestacks icon on your
desktop screen

Downloading and Installing the LGU Assessment Tool for COVID19 Data Collection App
1. Type the following URL in the
web browser’s address bar

ddc.doh.gov.ph/blhsd.apk

(The application will be


downloaded in your browser)

2. Click “blhsd.apk” to install

If the application is successfully installed,


you will see the Data Collect App icon on your
desktop screen

II. Accessing the LGU Assessment Tool for COVID-19 Response Data Collect App
1. Open/Launch the App

2. Enter your username and


password

3. Click the “SIGN IN” button

III. Encoding an LGU Assessment Report
1. Select the LGU level where
you want to encode a report
(i.e., City/Municipal or
Province)

2. Select the name of the LGU


from the dropdown menu

(i) For City/Municipality,


select the name of the
Province then the
Name of the
city/municipality

(ii) For Province, select the


name of the province

3. Click “Add”

A blank reporting form will


open. Encode the
assessment results and
ensure that all fields are
filled-up.

4. Ensure that you have
selected the correct date
of the last day of the
reporting period (e.g., for
June 1 to 15 report, select
June 15, 2021 or 2021-6-
15).

5. Click “Submit”

Note: The “Submit” button will


not be enabled if you have
blank fields (i.e., you have not
encoded a value in one on
more fields)

A summary report will be


generated for every successful
submission of an accomplished
form.

IV. Editing a Submitted Report Form

1. Click the Record ID


number of the report you
want to edit

2. Edit the previously


submitted data

3. Click “Update”.

Note: The system allows user


to edit a previously submitted
form within 7 days after the
initial date of submission.
