AFFIDAVIT OF ONE AND THE SAME PERSON

I, ANA ISABELITA LLANEZA VILLAROSA, Filipino, of legal age,

widow, with address at 103 C. Palanca Street, Legazpi Village, Makati City after
having been duly sworn to in accordance with law, do hereby depose and state that:

1. My name as it appears on my birth certificate is “Ana Isabelita Llaneza”.

2. However, I grew up using the name “Ana Isabel Llaneza” which appears in
most of my records and ID cards including those with PAG IBIG.
3. My married name is “Ana Isabelita Llaneza Villarosa”, which follows the
name appearing in my birth certificate.
4. I am one and the same “Ana Isabel Llaneza” and “Ana Isabelita Llaneza”.
5. I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose that this may serve.
